<p> TRIANGLE ORTHOPAEDICS VISION PLAN</p><p>Triangle Orthopaedics is pleased to announce that our voluntary vision benefit has been greatly enhanced. The new benefit has two plan options  Silver and Gold. Each option allows employees and their families to significantly reduce the expense of routine eye care. Note that the entire benefit now renews annually, and the co-payment for eyewear has been eliminated. In addition, the number of participating providers in Durham has increased from 11 to 63. Offered through Community Eye Care, the vision benefit includes the following:</p><p>Eye Examination</p><p> A routine eye exam once a year ($10 co-payment)</p><p> A contact lens fitting for new fits or re-fits, as needed ($10 co-payment) (Periodic contact lens evaluations are not covered)</p><p>Eyewear Allowance</p><p> An eyewear allowance every twelve months. The allowance is available at two levels: $100 (Silver Plan) and $150 (Gold Plan).</p><p>The allowance can be applied to frames, lenses, contact lenses, non-prescription eyewear, or any combination. So long as you select eyewear having a retail price less than or equal to your allowance, you incur no office charge for the eyewear. </p><p>HOW TO USE THE BENEFIT:</p><p>1. Select a provider from the Community Eye Care provider network (a complete list of participating providers can be found at www.communityeyecare.net).</p><p>2. Call the provider to make an appointment, and let them know that you have the Community Eye Care plan.</p><p>3. See the doctor and select your eyewear.</p><p>4.
