<p> Juvenile Court Transition Coordinator JOB DESCRIPTION</p><p>QUALIFICATIONS</p><p>A Bachelor’s Degree in Behavioral Sciences or Education preferred and two years related work experience. The ability to tactfully and efficiently communicate with others in person and in writing; a basic understanding and appreciation of human and social development; personal maturity to maintain composure under unusual stress or social interaction pressures; ability to learn the operations of the Iowa Courts and Juvenile Justice System. </p><p>REPORTS TO</p><p>1. Local Juvenile Court Services for client referrals and information exchange. 2. District Chief Juvenile Court Officer for monthly statistical reports and other information as necessary.</p><p>POSITION OBJECTIVES</p><p>1. Coordinate and facilitate YTDM team meetings to assist in engaging and empowering youth and families to be full participants in the meeting process and in the development of a service plan or transition action plan. 2. Utilize YTDM meetings to develop reentry transition case plans for youth that meet the youth’s individual needs and the state’s requirements for transition. 3. Assist the youth, his/her family and the school by arranging for school/community based services. 4. Serve as a resource for youth and families that request assistance with school, transition and family problems. 5. Assist in the development of appropriate programming resources to address the needs of at-risk youth. 6. Develop and coordinate partnerships, resources and services for transitioning youth from one educational setting to another. 7. Act as a liaison and advocate for Juvenile Court involved youth regarding educational and transition issues; consistently communicating youth progress to those involved, such as the court, the family, and schools. 8. Work collaboratively with school districts, social services, probation, and other community agencies to ensure educational and transitional supports and services are in place. 9. Collaborate with program staff and community agencies and participate in multi-disciplinary meetings to develop appropriate transition and educational plans. 10. Obtain and review youth’s educational records; facilitate smooth transition and timely school enrollment for youth by facilitating the transfer of youth records, including graduation requirements, transcripts, IEPs, standardize test scores and immunization records. 11. Monitor youth’s progress toward graduation requirements as well as educational and/or transition goals; assess factors that may affect youth’s progress; and identify and implement strategies to improve youth learning and attendance. 12. Assist the youth/family in obtaining and/or developing appropriate IEP’s/504 plans. 13. Provide tutoring assistance to increase youth homework completion, grades, and credits earned towards graduation. 14. Assist youth with exploring and/or enrolling in alternative high school diploma programs, such as the HSED. 15. Provide career/college exploration, including assistance with college visits, applications, registration/enrollment and completing financial aid. 16. Collect and utilize data for program improvement and development 17. Facilitate trainings regarding Juvenile Court involved youth and educational issues for staff and community partners; develop training materials. 18. Perform related duties similar to the above in scope and impact as required. </p><p>TRAINING The Juvenile Court Transition Coordinator shall attend all assigned training sessions.</p><p>PERFORMANCE EVALUATION A performance review shall be conducted annually by an official of the organization with input from JCS.</p><p>______-______-______Employee Date</p>
