<p> New Jersey Motor Vehicle Commission Replacement Dealer Plate Procedure Effective 8/1/03</p><p>When the dealership has misplaced a dealer plate(s) or suspects that a plate may have been stolen, the New Jersey Motor Vehicle Commission will issue replacement plate(s) to the dealership the following documentation must be sent to.</p><p>New Jersey Motor Vehicle Commission Business License Compliance Unit P.O. Box 171 225 East State Street, 4th Floor Trenton, NJ 08666</p><p> Letter addressed to the Motor Vehicle Commission, using the above address, indicating the plate(s) that is/are lost or stolen. Please be sure to include the plate number, as well as the sub-digit of each lost or stolen plate.</p><p> Copy of police report that lists each plate (including sub-digit) that has been reported as lost or stolen.</p><p> Check payable to “NJMVC” for $6.50 for each missing/stolen dealer plate.</p><p> Copy of current Motor Vehicle wall license.</p><p>During the Motor Vehicle wall license renewal period (January 1 through April), NJMVC cannot place any replacement/additional dealer plate orders. You must order the plate(s) prior to January 1 or after April l.</p><p>Dealer License-Replacement Plates (8-03) Dealer Plate Inventory Form</p><p>Dealership Name: Dealer ID #: (as listed on your wall license) (5 digit number)</p><p>Dealer Plate # Name of Individual Assigned to Plate Driver License # and State</p>
