Fire Marshal certificate if home is located in a dwelling containing more than 2 living units I. Home Study J. Completed Fire Safety Inspection Report K. Hot water checked (does not exceed 120 degrees) L. Copy of Evacuation Plan M Verification of completion of initial training; OR * statement of approval of training if not provided by DMR N. Verification of current CPR (card copied front and back) or statement from instructor noting type of CPR and date course passed O. Sample CTH Agreement for Services signed by * applicant(s) P.
