<p> SCHOLARSHIP APPLICATION INFORMATION</p><p>This scholarship is given to a son or daughter whose parent/guardian is a member in good standing of the Crossroads Officials Association. The parent/guardian of the applicant must have been a member in good standing of the COA for two consecutive years prior to the application date. Under an extenuating circumstance the committee may waive this requirement. The applicant should display character that exemplifies integrity, discipline, honesty, cooperation, determination, devotion, responsibility, understanding, and enthusiasm.</p><p>This application may be completed by the student before notification of acceptance to a post-secondary educational or vocational program.</p><p>Qualifications</p><p>1. Applicant must be a senior in high school who ranks in the upper 25% of his/her class or a college undergraduate who has a B (3.0) average or higher. 2. Applicant must be making definite plans to further his/her education in a chosen field. 3. Applicant must meet necessary entrance requirements in the institution of choice. 4. Applicant can be awarded this scholarship only once. 5. The parent/guardian of the applicant must have been a member in good standing of the COA for two consecutive years prior to the application date. (Under an extenuating circumstance the committee may waive this requirement.) 6. Three completed reference forms are required as per #13 of the scholarship application.</p><p>Amount of Scholarship: $1,000.00 Number of Scholarships: 2</p><p>The scholarship check will be made payable to the applicant, and may be utilized for any post- secondary educational need.</p><p>It is imperative that your application be completed with no omissions of any part of the application requirements.
