<p> CONNECTCARE PROVIDER NETWORK ACCESS</p><p>Below are instructions for employees’ that are Connecticut Residents with the choice of HMO and POS plan options. Those employees who are Massachusetts / Rhode Island residents have the choice of the FlexPOS Plan Options.</p><p>For CONNECTICUT RESIDENTS </p><p>How to Find a Doctor or Facility</p><p>1. Go to ConnctiCare.com 2. Under the ConnectiCare Logo on the left hand side click on “FIND A DOCTOR” 3. In Select a Plan Type, click on “All Other Plans/Don’t Know” for the HMO, POS or EPO options and click – Continue 4. For searching for a Primary Care Physician, click on “All Other Physicians, Hospitals, Labs, etc.” and click – Continue 5. You can search by the provider last name, facility name OR advanced search. The Advanced search allows you to search for a provider in a certain distance, specialty, affiliation etc. and click – Search. 6. From this page you will be able to view the list providers and from the top of the page you can email or print off the provider listing in a pdf format. </p><p>For MASSACHUSETTS and RHODE ISLAND RESIDENTS</p><p>How to Find a Doctor or Facility</p><p>1. Go to ConnctiCare.com 2. Under the ConnectiCare Logo on the left hand side click on “FIND A DOCTOR” 3. In Select a Plan Type, click on “FlexPOS Plans ” option and click – Continue 4. For searching for a Primary Care Physician, click on “All other Physician, Hospital, Labs, etc outside the State of Connecticut.” and click – Continue 5. From this page, you will be leaving the ConnectiCare Website and entering the MultiPlan website. Be sure that during your search, you remain in the Multiplan Network titled 'PHCS Healthy Directions'. Click on “I agree and wish to Continue. 6. Under the MultiPlan website click on “Doctor, Doctor or other Health Care Professional” and click – Continue. 7. You can search by location, type of provider, or the provider’s last name and then click – Continue. 8. From this page you will be able to view the list providers and from the top of the page you can email or print off the provider listing in a pdf format under “Printer Friendly”. </p>
