<p> INFORMATION SYSTEMS EXAMINATION BOARD</p><p>Application to become a Course Tutor</p><p>Before completing this form, applicants should read the explanation of the requirements to be satisfied for each position. The document is entitled ‘Staffing Definitions’ and can be found at www.iseb.org.uk.</p><p>In completing the answers to the sections below the following should be noted by way of explanation as to what is required.</p><p>ACCREDITATION SOUGHT</p><p>This should show which accredited qualification you wish to apply for, i.e. one of the following:</p><p>Foundation and Manager’s Certificate in IT Service Management Practitioner’s Certificate in IT Service Management ITIL Certificate in Infrastructure Management</p><p>Certificate in Freedom of Information Certificate in Data Protection</p><p>Certificate in Information Security Management Principles</p><p>Foundation Certificate in Software Testing Practitioner’s Certificate in Software Testing</p><p>Certificate in IT Architecture</p><p>Certificate in IT Law</p><p>Certificate in Software Asset Management Essentials</p><p>Foundation Certificate in Programme and Project Support Office Advanced Certificate in Programme and Project Support Office</p><p>Foundation Certificate in Project Management Certificate in Project Management for Information Systems</p><p>Certificate in IS Consultancy Practices</p><p>For Training Provider</p><p>This must state the Training Provider for whom you will be delivering the accredited training. If you are already approved for a particular qualification with another ISEB accredited Training Provider, please complete the form ‘Registration of Existing Course Tutor’.</p><p>ISEB/Form/Accred/0109/1.8 Copyright © The British Computer Society 2009 Position Applied For</p><p>This must state which position is being applied for. In the case of a Course Director, a dedicated form is available. In the case of a course tutor you must state if the application is for 1st or 2nd tutor; if neither option is chosen the application will be considered as one for 2nd tutor.</p><p>Working Experience in Qualification Area</p><p>This is a vital part of the application form as it asks for ‘your working experience in qualification areas’. Accordingly, you should provide a full detailed description of the practical experience you have here. Bullet points and high level generic references to your experience in this area of work is not enough. Sound, quantifiable experience is required.</p><p>NOTE: The required information must be provided here in full as well as an attachment of a CV. The information here must be enhanced by that provided in the CV. A duplication of same in another form is insufficient.</p><p>Experience as a Tutor</p><p>This should be completed as fully as the section on working experience. Do note that delivering presentations at seminars/conferences and internal briefings should not be included as this is not considered to be valid experience.</p><p>Academic/Professional Qualifications and Membership</p><p>You should list here all professional qualifications that you hold and that are relevant to the qualification area. You must include details of your qualifications which are required for the position you are applying for. Dates of achieving these qualifications will assist the processing of your application.</p><p>Any Other relevant Information</p><p>You should provide any further, relevant information which you think may assist your application.</p><p>Your Relationship with the Training Provider</p><p>You should state here the nature of your relationship with the Training Provider for whom you will be delivering the accredited training. The approval given by ISEB only relates to the Training Provider at the top of the form and if you wish to deliver training for another Training Provider, the ‘Registration of Existing Course Tutor’ form must be completed prior to delivering courses for this organisation.</p><p>Purchase Order Number</p><p>You should provide the reference for the purchase order which you are using to authorise payment for this application.</p><p>Signatures</p><p>You should now sign this application and arrange for the Course Director to sign it also.</p><p>ISEB/Form/Accred/0109/1.8 Copyright © The British Computer Society 2009 Failure to meet the requirements for the position applied for, and/or failure to provide sufficient information in support of the application will result in the application being rejected and forfeiture of the fee paid.
