Competency Assessment: Department Date: ______Observation Scenario: Pt has no allergies, is a full code status, has been admitted for abdominal pain, will go for an abdominal CT today. NPO, Abdomen is slightly distended/firm/BS diminished. Other assessment is negative. IV at 125 ml/h tubing is due to be changed in two days. IV clean, dry, intact, changed in the noc. Morphine 2mg IVP at 0300 for pain. Ambulate TID today. Critical Elements Validated (Staff can self assess by checking boxes to the left of each element) (how measured—date- initials) 1. On ADMISSION-RN explains the purpose and process for bedside shift report: “To keep you informed, we do an update at your bedside between the nurse going off duty and the nurse coming on. This assures that everyone has all the information needed to take great care of you. You don’t need to wake up for this, if you’re sleeping, we’ll be very quiet and try not to wake you.” On ADMISSION-RN explains use of white board, adds name, manages up self and adds physician in charge of case, manages up their skills. Orients patient and family to unit, explaining, call light, *cell phone use, overbed table Explains respect for privacy during report: “ Your privacy is important to us. We will ask visitors to step out during this discussion. If you are alright with them staying in the room, just speak up at the time and we’ll be glad for them to stay.” When the customer experience doesn’t go as expected, listens and responds with empathy and concern using ICARE model (Interest, Concern, Apologize, Respond, and Educate). Delayed response to call light Actively investigate service delays and initiates actions to resolve these delays. Explains these delays to patient, family, and visitors as needed. PRIOR TO END OF SHIFT- Prepare the patient for bedside report about an hour before (Acknowledge, Explain, Duration): “Mr(s) ____, I’ll be coming back in about an hour to do shift change report with the nurse who’ll be taking care of you next shift. We do this with each patient so we’ll just take a few minutes to review your care together. Is there anything I can do for you now? 3. (A-I-D-E-T, Key words, managing up, white board and notepad): DAY SHIFT “ Mr(s)____, Good morning. We’re here to do the nursing shift change report. This is ____ (nurse) and she/he will be taking care of you today. (Write name on white board and manage up). She/he is a (ex. very capable, terrific, really skilled etc. nurse). Assisting her/him as the nurse tech. will be ______(write name on white board and manage up). He/she will assist with your activities, bathing and meals. Identify patient using two identifiers (name and date of birth) and asks preferred name Wash hands upon entering room and use key words “I’m washing my hands for your safety” I’m going to take just a few minutes to review some important information about your care with (name the nurse) now.” To visitors: “For (patient name’s) privacy, would you mind stepping out of the room while we do this? (Patient can speak up and say “It’s OK for them to stay”.) Pull curtain if semi-private room. Review pertinent facts, observe skin/wounds/drains, run IV lines checking meds. Discuss pain management, activity level, nutrition and pending tests/procedures. Off-going nurse: “So the plan for today is …. (Record on white board - walking X times, eating X… , planning for discharge… using IS… etc.)” Patient is included: “Mr(s)___, is there anything you’d like to add? ” (Encourage use of pad of paper to write down questions for physicians etc.) Off going nurse thanks patient: “Thank you for allowing me to take care of you last night. I hope you have a good day today.” Oncoming nurse: “I’ll be back to talk with you more in about an hour. Please put your call light on if you need something in the meantime.” Nurse returns after report: “What can I do to make this a great day for you?” 4. (A-I-D-E-T, key words, managing up, whiteboard, and notepad): NIGHT SHIFT: “Good evening, Mr(s) ____, we’re here to do the nursing bedside report. This is ____ (nurse) and she/he will be taking care of you throughout the night. (Write name on white board and manage up). She/he is a (ex. Excellent, well experienced, great etc.) nurse. Assisting her/him as the nurse tech. will be _____ (write name on white board and manage up). She/he will be in to check on you several times during the night and will also take your blood pressure and temperature a couple of times. This is important to be sure you’re safe and doing well during the night. I’m going to review some important information now about your care with (the nurse).” Identify patient using two identifiers (name and date of birth) and asks preferred name Wash hands upon entering room and use key words “I’m washing my hands for your safety” To visitors: “For (patient name’s) privacy, would you mind stepping out of the room while we do this? Do you want to be awakened for pain (sleep, anxiety etc) medication or just let me know if you need something? “The lab may be in early in the morning to draw blood so the doctors will have the most up-to-date information to plan your care tomorrow, but hopefully you’ll be able to go back to sleep.” Off-going nurse thanks patient: “Thank you for allowing me to take care of you today Mr(s) ____. I hope you rest well this evening.” Oncoming nurse: “We’re here all night for you. Please call us for anything you need.” Nurse returns after report: “What can I do to make this a good night for you?
