<p>Dear Parent/ guardian:</p><p>Leicester Public Schools shall award a high school diploma or any other document connoting high school graduation only to a student who has satisfied the state Competency Determination requirement and who has met local graduation requirements.* To meet the state Competency Determination your child needs an Educational Proficiency Plan (EPP) because he/ she did not score 240 or above on his/her English language arts +/ or mathematics MCAS test(s). The attached Educational Proficiency Plan (EPP) includes:</p><p> a review of your student's strengths and weaknesses, based on MCAS and other assessment results, coursework, grades, and teacher input;  the courses the student will be required to take and successfully complete in grades 11 +/or 12; and  a description of the assessments the school will administer on a regular basis to determine if the student is moving toward proficiency. </p><p>Please contact the Leicester High School Guidance Department @ 508-892-xxxx if you would like an in- person meeting to discuss your child’s EPP. The plan outlines the academic requirements a student needs to fulfill to meet the state’s Competency Determination for high school graduation.</p><p>Student: SASID: Present Grade: Parent/ Guardian: Guidance Counselor: Date: </p><p>I. Required course(s): Students need to successfully complete the course(s) and demonstrate progress toward proficiency on an annual assessment. To measure progress toward proficiency a combination of scores of mid-term, final and unit exams will be used. The assessment will be a measure of the entire semester and based on subject matter standards. In order to show a student has met proficiency we will explore the use of the Department MCAS test designed specifically for EPP. III. Content area strengths & weaknesses: Assessment data, student interviews and teacher input will be accessed to detail student perceived and documented content successes and failures.</p><p>III. Post Secondary Vision: Using a self report or the Harrington O’Shea inventory the student’s preferences and interests, and the desired outcomes for post secondary education or employment will be revealed. </p><p>III. Additional academic supports / programs: Students will be encouraged to take advantage of the opportunities provided by the school to help them reach their post-secondary goals (e.g. tutoring, academic support classes, guidance workshops and college / career fairs). </p><p>IV. Communication by direct mail / e-mail and call notification: Parents/ Guardians are encouraged to communicate with the EPP team and set up an in-person meeting to discuss the details of the EPP. Evaluation: The EPP will be evaluated annually (a process that will include parent/ guardian input) and signed off by the principal to determine if all the requirements of the EPP have been met. </p><p>V.
