<p> Postgraduate Year 1 (PGY1) Residency Application Requirements</p><p>Applicants must be a Pharm.D. graduate of an accredited college of pharmacy and must be eligible for and obtain licensure in Colorado or another state. </p><p>In order to apply for the University of Colorado Hospital’s ASHP-accredited PGY1 Residency, the following materials must be submitted:</p><p>1. Letter of intent, not to exceed two pages 2. Curriculum Vitae 3. Official College of Pharmacy Transcript 4. Three Letters of Reference (free-text letter preferable); only the first three letters will be considered with the application</p><p>Application materials may be submitted all together in one packet or separately. There is no formal application form required.</p><p>All PGY1 applicants must participate in the ASHP residency matching program.
