<p> REQUEST FOR PROPOSALS FROM PRE-QUALIFIED GENERAL CONTRACTOR OFFERORS BRANDYWINE ELEMENTARY SCHOOL</p><p>The BOARD OF EDUCATION, FORSYTH COUNTY, GEORGIA, (the “Board”), pursuant to the provisions of the Georgia Local Government Public Works Construction Law, herein seeks competitive sealed proposals (“Proposal” or “Proposals”) from Pre-qualified General Contractors for the construction of Brandywine Elementary School to be located at 175 Martin Drive, Alpharetta, GA 30004. Any Offeror that intends to submit a Proposal is required to attend a mandatory Pre-Proposal Meeting, which will be held on the 22nd day of October, 2014, at 10:00 a.m. at the office of the Board at Professional Development Center, 1120 Dahlonega Highway, Cumming, GA 30040. The requirements for construction of Brandywine Elementary School and the duties and responsibilities of the General Contractor whose Proposal is accepted, are set forth in the following documents and things: this Request For Proposals (“Request”); the Instructions to Offerors; the Proposal Form; the Standard Form of Fixed Price Construction Contract (the “Contract”); the form of Bid Bond; the form of Payment Bond and Performance Bond; the drawings; the specifications; and, any addenda issued by the Architect (all of which are collectively referred to hereinafter as the “Contract Documents”). General Contractors interested in submitting Proposals must obtain copies of the Contract Documents from the office of Lindsay Pope Brayfield Clifford & Associates, Inc. (the “Architect”), 344 West Pike Street, Lawrenceville, GA 30046 (770) 963-8989. The Contract Documents may be obtained electronically from the Architect by E-mail request to [email protected]. General Contractors will be given access to the Architect’s ftp site where the Contract Documents may be downloaded at no charge. Plans may also be accessed by Subcontractors or Material Suppliers. Any General Contractor submitting a Proposal must procure a set of the Contract Documents from the Architect. The Proposal Form must be fully completed, signed and notarized, and timely submitted. The Contract Documents are available for inspection by the public at the office of the Architect identified above and the offices of AGC Builders Exchange Plan Room Repro Products, 4485 South Atlanta Road, Smyrna,</p><p>1 GA 30080, 678-298-4130; Reed Construction Data, 30 Technology Parkway, Norcross, GA 30092 (770) 417-4000; AGC Plan Room, c/o Gainesville Whiteprint, 312 Bradford Street, Gainesville, GA 30501 (770) 536-8254; LDI, 558 Old Norcross Road #105, Lawrenceville, GA 30046 (770) 237-9363; and, McGraw-Hill Online Plan room at http://www.construction.com/ProjectCenter/. The Contract Documents require, among other things, the furnishing of all materials, labor and equipment for construction of Brandywine Elementary School. The Board reserves the right to make available other relevant documents or information, if any, concerning Brandywine Elementary School. Any Proposal submitted in response to this Request should comply strictly with all requirements set forth in the Instructions to Offerors. Any such Proposal must contain the completed Proposal Form setting forth the General Contractor’s proposed lump sum contract price for full and complete construction of Brandywine Elementary School in conformity with all requirements of the Contract Documents. The Contract will be awarded based on the evaluation factors identified below. Any Proposal must include a fully executed Bid Bond in the amount of five percent (5%) of the proposed lump sum contract price (exclusive of alternates and unit prices) in the form required by the Instructions to Offerors. Note that the Proposal Form requires, among other things, Offerors to disclose information concerning specific proposed subcontractors for the site work, fire protection, electrical, plumbing, mechanical, structural steel erection, structural steel fabricator, masonry, and roofing subcontractors. For any work requiring a specialty or professional license, only licensed subcontractors may be submitted for consideration, and copies of all applicable licenses shall be attached to the Proposal Form. The Board will rely upon a General Contractor’s identification of proposed subcontractors and the submission of information related thereto. The Board reserves the right to request that any specific proposed subcontractor(s) be changed. In evaluating Proposals, additional information may be sought and obtained from any General Contractor concerning such General Contractor’s Proposal or its proposed subcontractor’s qualifications to construct Brandywine Elementary School. The Board intends to award the Contract to the responsible and responsive General Contractor whose Proposal is determined in writing to be the most advantageous</p><p>2 to the Board, taking into consideration the following evaluation factors and their relative importance based on the point system ascribed below:</p><p>(A) BASE PROPOSAL OF BRANDYWINE ELEMENTARY: 20 Points (B) OFFEROR OVERVIEW: 15 Points (C) RELATED EXPERIENCE: 20 Points (D) QUALIFICATION EXPERIENCE OF PERSONNEL: 20 Points (F) QUALIFICATION / EXPERIENCE OF SUBCONTRACTORS: 15 Points (E) OFFEROR’S EVALUATION: 10 Points</p><p>The Board has resolved that the Director of Facilities shall administer the competitive sealed proposal process as provided in that certain Resolution adopted by the Board on August 21, 2014, including, without limitation, the preparation and advertisement of this Request, receiving and opening proposals, evaluating proposals, engaging in discussions, negotiations and revisions of Proposals with Offerors found by the Director of Facilities to be reasonably susceptible of being selected, and making recommendations to the Board for an award of the Contract or other disposition of the Proposals as may be appropriate. All Proposals must be submitted and received in three parts. </p><p> Part One, consisting of Section A-1 through A-5 of the Proposal Form must be (1) fully and properly completed, signed and notarized, (2) submitted in a sealed envelope, (3) any last-minute price adjustment shall be submitted in a sealed envelope clearly noting either addition or decrease, a dollar value, and a signature of the offeror’s agent that is authorized to make the last-minute price adjustment; and. (4) received by the office of the Facilities Department of the Board located at Professional Development Center, 1120 Dahlonega Highway, Cumming, GA 30040 no later than 2:00 p.m. on the 13th day of November 2014. This envelope will not be opened until after the evaluation of Parts Two and Three. </p><p> Part Two consisting of Sections B through E of the Proposal Form must be (1) fully and properly completed, signed and notarized, (2) submitted in a sealed</p><p>3 envelope, and (3) received by the office of the Facilities Department of the Board located at Professional Development Center, 1120 Dahlonega Highway, Cumming, GA 30040 no later than 2:00 p.m. on the 13th day of November 2014.</p><p> Part Three, consisting of Section F of the Proposal Form, for Site Work Subcontractor, Fire Protection Subcontractor, Electrical Subcontractor, Plumbing Subcontractor, Mechanical Subcontractor, Structural Steel Fabricator Subcontractor, Structural Steel Erection Subcontractor, Masonry Subcontractor, and Roofing Subcontractor must be (1) fully and properly completed and signed, (2) submitted in a sealed envelope, and (3) received by the office of the Facilities Department of the Board, located at Professional Development Center, 1120 Dahlonega Highway, Cumming, GA 30040 no later than 5:00 p.m. on the 13th day of November 2014. The Director of Facilities, or his delegates, will receive such Proposals at such place and times. If either Part One, Part Two, or Part Three is not received by the office of the Facilities Department of the Board by the designated time specified above for each part, the entire Proposal will be disqualified. No Proposal, or part thereof, shall be opened by the Director of Facilities until after 5:00 p.m. on the 13th day of November 2014. At the discretion of the Director of Facilities, but in conformity with the applicable provisions of Georgia law, Offerors found by the Director of Facilities to be reasonably susceptible of being selected may be afforded an opportunity for subsequent discussions, negotiations and revisions of Proposals for purposes of pursuing and obtaining best and final offers. The Board reserves the right to reject any or all Proposals and to waive any technicalities or informalities. Any Proposal submitted in response to this Request shall remain open for acceptance by the Board, and same shall be honored by the Offeror, for a period of sixty (60) days from the date set forth hereinabove for the receipt of Proposals. All anticipated federal, state and local permits required for the project have not been obtained. Required permits are anticipated on or before December 12, 2014. The Contractor shall accomplish Substantial Completion of the Project on or before April 1, 2016.</p><p>4 All rights of way and easements anticipated for the Project have not been obtained, but it is anticipated that all rights of way and easements will be obtained prior to Site Development Permitting and should not affect delivery of the Project. Any questions or comments concerning this Request should be addressed in writing to Mr. Bill McKnight, Director of Facilities, Forsyth County Board of Education, 1120 Dahlonega Highway, Cumming, GA 30040.
