<p> Daycare Consent and Agreement</p><p>Client’s Name ______</p><p>Pet’s Name(s) 1) ______2) ______</p><p>Daycare at Four Seasons Animal Hospital is a service we are glad to provide for our clients. We must keep in mind that this facility is an Animal Hospital that provides Daycare service, and not just a kennel. In order to establish a safe and healthy environment for all participants in its daycare program, we require the following preventative medicine measures to be current to protect daycare animals from infectious disease:</p><p>Vaccinations: DOG: DHPP – Distemper, Hepatitis, Parainfluenza and Parvo Rabies Bordetella – Kennel Cough Bacteria Influenza – Canine H3N2 Flu Fecal Exam – Intestinal Parasites</p><p>CAT: FVRCP – Distemper, Respiratory Virus, Calicivirus and Pneumonitis Rabies Bordetella – Kennel Cough Bacteria Fecal Exam – Intestinal Parasites</p><p>External Parasites: FLEAS & TICKS are contagious diseases. On admission your pet will be examined for external parasites. If any are found, we will medicate your pet at your expense before Daycare.</p><p>Sickness: Should a condition develop or become apparent while your pet is at Daycare, we will treat that condition at the owner’s expense, if the condition warrants immediate attention.</p><p>Reproduction: Because Daycare involves play with other animals it is required that all animals in Daycare be neutered: spayed (female) or castrated (male). Un-neutered animals will not be allowed to play in Daycare with other animals.</p><p>I verify that the above named dog(s) are in good health and to my knowledge have not shown clinical signs of any communicable disease within the last 14 days. I also certify that neither dog has caused harm to nor shown aggressive or threatening behavior towards people or dogs.</p><p>Special Pricing: Pre-payment of at least 5 days of a Daycare stay will yield a $2.00 discount* for each day. *Daycare while boarding is excluded from this pre-pay discount plan.</p><p>I understand that attendance by my dog(s) or cat(s) at this Daycare program involves group play with other pets. Although the staff at this facility will supervise all participants, I accept that play behavior, unknown or undocumented aggression, or participation in routine daily activities can lead to altercations or injuries. I assume the risks of and responsibility for the costs to treat any injuries my dog(s) or cat(s) sustains while playing at this facility.
