<p>Business Process Document: Employee Self Service: Change Your Name</p><p>In HCM:  Click the Self Service link  Click the Personal Information link  ( the system displays your current name)  Enter the date in which the name change is effective. The date can be in the past or future. Click the Date Calendar button.  Click the desired date  Click the Edit Name link  Enter the appropriate name change  Click the OK button  Click the Submit button  Click the OK button  The system displays the new name format</p><p>Please note: employees must have an updated copy of their social security card & be prepared to show it to their supervisor prior to submitting the request via ESS. Hospital EAST asks that after the supervisor verifies the new name on the social security card, the supervisor can email the EAST team (listing the name as it appears on the card) to give permission to actually make the name change in HCM. </p><p>In addition, if the employee is a Licensed Nurse, their name cannot be changed unless their Texas Nursing License has also been revised to reflect the name on the social security card. Both items must be verified by the Nurse Manager prior to giving EAST permission to change. </p><p>The name change should appear in the on-line directory the next day. At that point, the employee may go to Campus Police to obtain a new badge.</p>
