A ‘Notification’ form is available on the CSSIW Internet site or from your CSSIW regional office.</p><p>This form includes a list of all the information you must tell CSSIW about.</p><p>Page 22 of 53 08/2013 Part 2 Meeting the regulations and standards</p><p>Introduction</p><p>This section of your application is your opportunity to show that you have understood the principles and requirements expressed in the Regulations (The Child Minding and Day Care (Wales) Regulations 2010), and the National Minimum Standards for Regulated Child Care (NMS). The following information is intended to help you to make sure that fire safety matters have been dealt with by you before the CSSIW inspector visits your home. Please have this form available when the inspector visits your home. 2. Applications for the registration of a child minder will not normally be referred to the fire service unless overnight care is to be provided. 3. All the boxes below should be completed and this will help the inspector make a judgement as to whether they need to make contact with the fire service. 4. Should an inspection reveal matters of concern regarding fire safety, CSSIW will contact the local fire service for discussion. This will be used to provide up to date information to the public about child care provision in the area. </p><p>On the granting of registration, this would routinely include: 5. Your name. 6. Your address and the address of the premises if this is different. 7. Other contact details e.g. telephone number or email address. 8. Any unique reference number used by CSSIW in respect of your registration. 9.
