
<p>Checklist for Application for Services for Continuing Education and Driver Education Students</p><p> Make an appointment with Disability Support Services (DSS) for a meeting with the ADA Coordinator or DSS Coordinator → Call DSS La Plata Campus: (301) 934-7614 Leonardtown Campus: (240) 725-5420 Prince Frederick Campus: (443) 550-6009</p><p> Complete "Request for Accommodations for Continuing Education and Driver Education Students" form </p><p> Read and sign “Rights and Responsibilities of Students with Disabilities” form </p><p> Read and sign “Rights and Responsibilities of the College of Southern Maryland” form</p><p> Read and sign “Procedure for Requesting Services” form </p><p> Provide appropriate documentation of your disability from your medical professional. If your documentation is determined insufficient, you can meet with the ADA coordinator to determine appropriate and reasonable temporary accommodations</p><p> Discuss the impact of your disability on the learning process and discuss appropriate and reasonable accommodations with the DSS staff</p><p> Contact DSS to review and pick up your accommodations plan</p><p> Have all faculty sign the master accommodations plan and give them a copy</p><p> Return master accommodations plan to DSS with all faculty signatures and your signature</p>
