Tel: 01793 872800 Headteacher: Mrs Karen Pyman Company Number 8713217</p><p>The Hub - Breakfast and After School Club</p><p>The Hub at Millbrook Primary School provides high quality, affordable and accessible childcare for children aged 4 – 11 years in a safe and supportive environment both before and after school. The emphasis is on creative fun – we want your child to come here and enjoy it. Staff Our Hub Play Leaders are Jessica Brockbank, who has a CACHE level 3 diploma in Child Care and Education and is a Teaching Assistant at Millbrook and Dominique Smiles, who is also a Teaching Assistant. Our Play Assistants are Jewels Burrows , Megan Thornett and Kerrie Shaw who are also Teaching Assistants in the school. All staff have enhanced DBS checks. Meals Breakfast and /or tea is provided for all children. Toast, cereals and fresh fruit together with a drink and occasionally a cooked breakfast is available in the morning and a healthy snack (or occasionally a cooked meal) is provided after school. Location The Hub is based in the community area of the school. Staff will ensure your child reaches their class safely each morning and collect them from the class if they are attending after school. Registration Please complete a registration form and update us with any changes to ensure we have your most up to date details. Children may be dropped off at the breakfast club from 7.45 am onwards. If your child is staying after school they can be collected any time before 6.00pm. An adult must drop off or pick up a child. Fees Breakfast Club - £3.00 per session After School Club - £7.50 per session</p><p>Please book and pay in advance on Parent Pay</p><p>Achieving Excellence, Fulfilling Potential The Hub - Breakfast and After School Club</p><p>Terms and Conditions I consent for my child to attend The Hub, I understand that The Hub has policies and procedures and that there are expectations and obligations relating to both The Hub, myself and my child and agree to abide by them.</p><p>I give permission for a trained member of staff to administer appropriate first aid if required</p><p>I give permission for The Hub to seek any necessary emergency medical advice or treatment in the event that my child is involved in a serious accident |I expect to be contacted immediately on the above telephone numbers.</p><p>Persistent late collection of my child may result in a charge of £2.50.
