Language must not be exculpatory. 7 Identification of funding source(s). 8 Identification of whom to contact (a) on research team with questions, concerns, complaints or about a research-related injury; and (b) at the IRB for same issues and information about research subjects' rights. 9 Statement that participation is voluntary and that the subject may discontinue at any time. Refusal to participate or withdrawal will not involve a penalty or loss of benefits to which the subject is otherwise entitled. 10 The information in the form is given in a language likely to be understandable by the subject or the subject’s representative 11 The form includes no exculpatory language through which the subject or the subject’s representative is made to waive or appear to waive any of the subject’s legal right, or releases or appears to release the investigator, the sponsor, the institution, or its agents from liability for negligence.
