This Shadowing Experience Is Meant to Provide the Student with a Meaningful, Real-World

This shadowing experience is meant to provide the student with a meaningful, real-world experience to see what is involved in your profession on a daily basis. The student is earning credit for his/her Sports Medicine class at Heritage High School; therefore it is important that any unacceptable behavior is not accounted for in their log. Students will be journaling their experience without using the real names of patients (as to not violate confidentiality) so that they may chart what they have learned as well as reflect on their experience. Thank you for your time and willingness to promote your profession to our students. If you have any questions or concerns please contact Ms. Davies, ATC.

Name of Student ______Name of Supervisor ______

Profession ______Organization Name ______

Please rank from 1 (Poor) to 5(Outstanding) t i r s e e n l t r o u I v a i

a u i v O n t n o

D a a o g i n i h i H t s

g e i S l i s n B a e S I t f o o r T P

m Signature s Shadowing/ Internship requirements - Arranging the hours o It is your responsibility as the student to determine the location that you would like to shadow and setting it up. . Contact the person in charge . Arrange a time to sit down for an information/ interview session . Determine days and times - If you are concerned about your ability to get these done throughout the year you must contact Ms. Davies ASAP (after spring sports start is too late)

- Paperwork o Each week you will need to turn in your hours to be recorded. o Hours turned in more than 2 weeks late will not count towards your total. o All hours need to be signed by your supervisor - Scheduling o Make sure that the times that you shadow are convenient for you as well as the business. o Be consistent! Have a regular time and day that you shadow, don’t make your supervisor guess when you will be coming in. o Be on time. Make sure that you will be able to make it slightly early to your site each time. If you cannot, then try to move it. - Behavior o You are expected to be . Helpful  If you are bored ask questions and/or ask if there is anything you can do to help out. . Attentive  Pay attention to what is going on around you. Try to make connections to what is learned in class.  Even if you are bored DON’T SHOW IT! . Professional  Make sure your appearance shows you care. Wearing nice pants and shirt shows that you are not just some “lazy high school student”  Use appropriate language o No use of “hecka” or other slang in a professional setting. - Signatures o In order for your hours to count your supervisor must sign off.

- No Shows – for on campus sports only o If you sign up for a game or practice and do not show up, 4 hours for a game or 2 hours for a practice will be subtracted from your total (you are taking that opportunity away from someone else that wanted to be there if you don’t show up). o If you are absent to school on a day you are scheduled to work you need to text/ email Ms. Davies when you call into school for your absence or before lunch when at all possible. (Call in sick for your hours AND for school)

- Time Sheets o Are collected every Thursday/ Friday (the last day you have class every week). o Hours not reported within 2 weeks late will not be accepted

- If a student chooses to do a summary paper on his/her experiences in the internship this will be considered extra credit and will be worth up to 30 points.
