<p> MEMORANDUM OF UNDERSTANDING</p><p>This MEMORANDUM OF UNDERSTANDING (MOU) is signed at Ahmedabad on this 18th day of September, 2003, between Self-Employed Women’s Association, having its registered office at SEWA Reception Center, Opposite Victoria Garden, Ahmedabad, represented by Mirai Chatterjee, Co-ordinator SEWA Social Security, and Kachchi Jain Seva Samaj, opposite Navchetan high school, Paldi, Ahmedabad, represented by Shri Chandrakantbhai ______, and it binds both parties to comply with the terms of the agreement.</p><p>Whereas both Kachchi Jain Seva Samaj and SEWA have decided that the Samaj will provide Antenatal Services to the members of VimoSEWA of Ahmedabad city which are as follows : </p><p>1. Four Antenatal checkups which would include : Blood pressure, weight, fundal height, abdominal checkup and internal checkup whenever required. 2. Two Tetanus Toxoid injections 3. Iron, folic and calcium tablets 4. Laboratory investigations : Blood test – blood group, haemoglobin and sugar & Urine test – sugar, albumin. 5. Health education and counseling </p><p>The terms and conditions, for providing the above mentioned services, agreed upon by both the parties after mutual discussions are as follows:</p><p>TERMS AND CONDITIONS </p><p>1. The Kachchi Jain Seva Samaj will use its infrastructure and resources to provide all the services under the project.</p><p>2. The Kachchi Jain Seva Samaj shall provide the above mentioned services through its hospital at Navchetan Medical Centre, opposite Navchetan high school, Paldi, Ahmedabad.</p><p>3. The period of this project is from 18th September to 31st December, 2003 which is extendable if both the parties to this MOU mutually agree to it. Those VimoSEWA members who have approached the Trust for availing benefits under this project on 31st December, 2003 shall be eligible to receive the entire package of services during her pregnancy period.</p><p>4. The services under the project shall be provided by the Kachchi Jain Seva Samaj Navchetan Medical Centre for Rs. 200 per pregnant member of VimoSEWA. This amount shall be paid to the Samaj by SEWA on behalf of VimoSEWA as advance by account payee cheque. </p><p>5. The Samaj shall maintain a record of expenditure incurred against the advance payment made by SEWA on behalf of VimoSEWA and provide a monthly statement of expenses to VimoSEWA. 6. The first instalment of advance payment shall be Rs. 2000 @ Rs. 200 per VimoSEWA member. The following instalments during the project period shall be made after adjusting against the balance remaining from the first instalment. </p><p>7. At the end of the project, the Samaj shall repay the remaining balance amount if any to SEWA and if there is any amount outstanding in the name of SEWA, it shall be paid to the Samaj.</p><p>This MOU is signed by the parties below as a mark of their acceptance of the terms of the agreement and their mutual obligations.</p><p>VimoSEWA Kachchi Jain Seva Samaj Ahmedabad Ahmedabad</p><p>Mirai Chatterjee Mr. Chandrakant Co-ordinator Trustee SEWA Social Security </p>
