Mr. Christensen made a motion, seconded by Ms. Guilbert to approve the minutes. The motion was unanimously carried.</p><p>OLD BUSINESS</p><p>Review of Form for HAD Supervisor</p><p>Mr. Christensen made a motion, seconded by Ms. Guilbert to table this review until our next Board meeting. The motion was unanimously carried.</p><p>NEW BUSINESS</p><p>Review of Permanent Audiologist Applications for Licensure</p><p>Ms. Guilbert made a motion, seconded by Mr. Christensen to approve the following applications for licensure:</p><p>Michelle D’Amico Rebecca Germano Gina Giordano Edward Keels Jung Hwan Lee Elena Zamora</p><p>The motion was unanimously carried. </p><p>Review of Permanent Hearing Aid Dispensing Applications for Licensure</p><p>Ms. Guilbert made a motion, seconded by Ms. Courtright to deny the following applications for licensure. Applicants will need to apply for temporary license until they take the Delaware Hearing Aid Exam.</p><p>Michelle D’Amico Rebecca Germano Gina Giordano Edward Keels Jung Hwan Lee</p><p>The motion was unanimously carried.</p><p>Review of Permanent Speech/Language Pathology Applications for Licensure</p><p>Dr. Michelli made a motion, seconded by Ms. Guilbert to propose to deny the following application for unlicensed practice:</p><p>Karen Bennett Board Speech Pathology, Audiology and Hearing Aid Dispenser Minutes – October 10, 2007 Page 3</p><p>The motion was unanimously carried.</p><p>Ms. Connolly-Gaskin made a motion, seconded by Ms. Courtright to approve the following applications: Cleavonne Stratton Sarah Venable Cheryl Nelson Elizabeth Merrick Rachel Stankovits</p><p>The motion was unanimously carried.</p><p>Review of Temporary Speech/Language Pathology Application for Licensure</p><p>Ms. Connolly-Gaskin made a motion, seconded by Mr. Christensen to approve the following application:</p><p>Kelly Diamond</p><p>The motion was unanimously carried.</p><p>Review of Continuing Education Activities </p><p>Ms. Courtright made a motion, seconded by Ms. Bilton to approve the following continuing education for the hour and category shown: Sponsor: Christiana Care Licensee: Kristen Palmer Activity: Learn to Tango Contact Hours: 1 hour Category: 1 hour for Clinical Skills </p><p>The motion was unanimously carried.</p><p>Ms. Courtright made a motion, seconded by Ms. Bilton to table the following continuing education for the hours and category shown:</p><p>Sponsor: Department of Health & Social Services of DE Licensee: Susan Berry Activity: Spelling – Adults on the Autism Spectrum: Success in the Community and the Workplace Contact Hours: 14.5 hours </p><p>Ms. Courtright made a motion, seconded by Mr. Christensen to approve the following continuing education for the hours and categories shown:</p><p>Sponsor: Birth to Three Early Intervention System, DE Health & Social Service Board Speech Pathology, Audiology and Hearing Aid Dispenser Minutes – October 10, 2007 Page 4</p><p>Activity: Carolina Curriculum for Infants & Toddlers with Special Needs Contact Hours: 3.5 hours Category: 3.5 hours Clinical Skills</p><p>Sponsor: DSHA Licensee: Sheree Burke Activity: Enhancing Phonological & Metaphonological Skills of Children with Highly Unintelligible Speech Contact Hours: 6.0 hours Category: 6.0 hours Clinical Skills</p><p>The motion was unanimously carried.</p><p>Review Audit for Continuing Education from licensees</p><p>Dr. Michelli made a motion, seconded by Mr. Christensen for the review of the Audit for Continuing Education. The Board reviewed continuing education documentation of those licensees that were selected for random audit after the July 31, 2007 license renewal. Licensees who were deficient in their continuing education will receive a letter instructing them to submit an explanation and/or documentation of additional hours.</p><p>The motion was unanimously carried.</p><p>Other Business before the Board</p><p>Ms. Wagner went over the schedule for 2008. Dr. Michelli made a motion, seconded by Ms. Connolly-Gaskin to approve the 2008 schedule for Board Meetings.</p><p>The motion was unanimously carried.</p><p>Public Comment</p><p>There were no public comments.</p><p>Next Scheduled Meeting</p><p>The next meeting will be held on Wednesday, November 14, 2007 at 2:00 Conference Room B, second floor of the Cannon Building, 861 Silver Lake Boulevard, Dover, Delaware. </p><p>Adjournment</p><p>There being no further business, Ms. Guilbert made a motion, seconded by Ms. Connolly- Gaskin to adjourn the meeting. The motion was unanimously carried.
