<p> Bees Club Booking Form (After School Club) Autumn 1 Dear Parents and Carers,</p><p>If you would like your child/children to come to the After School Club could I please ask that you book a half term in advance as the club has become very busy and we can not always guarantee a place for your child.</p><p>Please complete and return the form below to the office, you will need to pay online on ParentPay, but we must still have your booking form. We also accept childcare vouchers. If you have any queries please do not hesitate to contact me at the school office. </p><p>The charges are: £9.00 per day 1st child, £8.00 per day 2nd or additional children</p><p>Payment method: ParentPay [ ] Vouchers [ ]</p><p>Once you have sent in your half termly booking form and the places have been registered, you will no longer be able to cancel sessions and be refunded. You will be able to swap a session within the half term, but only if a place is available on the date you require. If you wish to cancel or swap we still need 48 hours’ notice.</p><p>Can you please indicate if your child/children needs to be collected from any clubs that run after school and taken to the After School Club.
