<p>ACCESS CODE POLICY</p><p>Savage School District</p><p>1. Must be an adult resident of the community (18 years old) to be eligible for access code.</p><p>2. Each code holder will submit a non-refundable $10 deposit to the district.</p><p>3. Savage School students are not eligible to receive and access code- including 18 year olds.</p><p>4. Each user or representative of a non-school group must complete and sign a school district Facility Use agreement form (4330) prior to using the facilities or receiving an access code. Facility Use forms are available in the school district office.</p><p>5. Access codes must be only used by the registered use. Guests may accompany the user. Registered Access Code user is responsible for their guest- both adults and students.</p><p>6. In accordance with Facilities Use Policies, the use of school district premises by non-school or organizations requires a representative of the group who is a registered Access code holder to sign up for specific times of use. Contact school district office for sign up prodedure.</p><p>7. Misuse of Access Code will result in the invalidation of Access Code and loss of access to Savage School District facilities.</p><p> First Offense-Formal notification of policy violation and invalidation of Access code for 2 weeks.</p><p> Second Offense- Formal notification of policy violation and invalidation of Access code for 6 months.</p><p> Third Offense- Formal notification of policy violation and invalidation of Access code. BOARD APPROVAL REQUIRED FOR RENEWAL OF ACCESS CARD.</p><p>8. Inform school district office if Access Code is lost or stolen.</p><p></p>
