<p> TO APPLY FOR RESIDENCY AT THE VA WESTERN NY HEALTHCARE SYSTEM </p><p>Deadlines All required materials must be in receipt by December 31 to be considered for an interview for the PGY1 Program and PGY2 programs. The PGY2 programs reserve the right to extend the deadline.</p><p>Applicants are requested to complete an onsite interview prior to entering into the matching process Please note that even though we are in receipt of all materials, an interview is not guaranteed.</p><p>RESIDENT SELECTION CRITERIA Resident applicants are required to upload to PHORACAS/Webadmit the following: https://portal.phorcas.org/</p><p>1. A letter explaining their desire to participate in the residency program 2. Proof of US Citizenship and a picture ID 3. Their most current transcript from an ACPE accredited College of Pharmacy with proof that the candidate will matriculate with a Doctor of Pharmacy degree 4. Three letters of recommendation from clinical colleagues/preceptors using the PHORCAS /Webadmit format.</p><p>THE VA APPLICATION IS NOT REQUIRED</p>
