Verify that each Cadet is physically/mentally qualified to take part in vigorous activities, or that all disqualifying physical/medical/disability limitations have been disclosed. If limitations exist, then confirm an ADA Accommodation has been granted in accordance with the Medical References Manual.</p><p>4b. Cadet must meet the guidelines contained in the Physical Readiness Manual, which provides minimum standards for attendance at Recruit and Advanced Training. This will be annotated on the Request for Training Authority (NSCTNG 001) prior to submission. </p><p>4c. Obtain parental certification in the Request for Training Authority (NSCTNG 001) that the Cadet is neither under a physician’s care nor taking prescription medications OR if so, the Medical History Supplemental must be completed by parent and medical provider and submitted to the COTC.</p><p>4d. Ensure that the Cadet has been enrolled in the NSCC/NLCC for a minimum of six weeks prior to the start of Recruit Training/NLCC Orientation. For an Advanced Training, verify Cadet has successfully completed Recruit Training/NLCC Orientation.</p><p>4e. Ensure that the Cadet meets the age and/or rate requirements for the requested training prior to submission.</p><p>4f. Verify each Cadet has completed the academic requirements (correspondence course, etc.) prior to reporting for training.</p><p>4g. Cadet must have all seabag items required on the list provided by the COTC. COs should contact COTC if seabag list has not been provided.</p><p>4h. Brief cadet and parents on the specifics of the requested training. Include transportation policies, reasons that Cadet may be returned home early, transportation costs to the parents if the Cadet is returned home, training content, and the length and overall cost of the training.</p><p>4i. Verify that the identification card is in good condition and will be current through the end of the training period.</p><p>4j. Process registration in Magellan and forward the cadet’s properly completed and endorsed Request for Training Authority (NSCTNG 001) with the required deposit to the COTC or other individual as listed in the training schedule.</p><p>NSCTNG 013 (REV 12/14) PREVIOUS EDITION IS OBSOLETE TRAINING CHECK-OFF LIST VERIFYING 5. TRAINING REQUIREMENTS (PRIOR TO DEPARTURE FOR TRAINING) OFFICER</p><p>5a. Sign the Training Authority (NSCTNG 005) certifying that the Cadet is physically qualified prior to issuing the cadet orders for Training. Cadets will be returned home if any required item is missing from the service record.</p><p>5b. Conduct a Seabag inspection. Be sure to confiscate, and return to the Cadet’s parents or guardians, all contraband and unnecessary items.</p><p>5c.
