<p> Welcome to Online Enrollment for Your 2018 Benefits!</p><p>You can now enroll online for your benefits for 2018 through the McDonald’s Licensees and Ronald McDonald House Charities® Health & Welfare Plan. This self-serve system, available 24/7, makes looking up benefit information and enrolling online easy. Follow these instructions to access the website, log in and enroll. </p><p>Annual Enrollment is from November 6 through December 20.</p><p>Make sure you log in and complete your enrollment on time! </p><p>Get Started Go to www.MyMcDBenefits.com — the site has been updated to be your one-stop for all benefits information! This mobile-friendly site helps you make the most out of your benefits, and will be regularly updated with tips and videos throughout the year! When you’re ready to enroll for 2018 benefits, simply click “Enroll” to go to the enrollment site, where you will need to log in:</p><p> o Username (EID): The first 2 letters of your first name + last 4 digits of SSN + MMDDYY of your date of birth (enter the letters as upper case) o Password (PIN): Your initial password is the four (4) digits of the year of your birth (YYYY). You will have the option to change your password when you begin your enrollment. Example: First name: John, SSN: xxx-xx-1234, date of birth: 06/05/1975 EID: JO1234060575 PIN: 1975 If you use your middle name, your login may be the first letter of your first name and the first letter of your middle name. Example: A. Connor Smith would be AC. Need help logging in? Call the Participant Support Line at (866) 962-2303. Representatives are available Monday – Friday, 7 am to 7 pm Central Time (CT). Enroll After logging in, you will have the opportunity to review your benefit information for the upcoming year (2018). After you have reviewed your available options, click “Select 2018 Benefits” to enroll. You will be guided through several screens where you will elect the benefits you wish to be enrolled in for the 2018 plan year. Take note: Your elections will not be recorded and saved until you complete all of the screens and reach the Confirmation page. Don’t forget to write down your confirmation number and keep it for your records. If you call the Participant Support Line for help, you will need to provide your confirmation number. You can return and make additional changes to your elections at any time before December 20, 2017. Each time you make a change, you will receive a new confirmation number. Remember to write down your confirmation number each time. Only the last confirmation in the Annual Enrollment period will be processed. </p><p>Have questions about your benefits? Talk to a Health Pro at Compass who can help you understand the benefits and decide what you want to have next year! Just call (800) 941-9260 (Monday – Friday, 8 am – 8 pm CT). After You Enroll You can access and print a copy of your confirmation statement online. You also will receive a confirmation statement in the mail at home in late December. When you receive your statement, check it carefully to make sure everything looks accurate. If there are any issues, please contact your employer right away. And remember, you can visit www.MyMcDBenefits.com at any time during the year to get information about your McDonald’s Licensees and Ronald McDonald House Charities Health & Welfare Plan benefits. The site is mobile- friendly and no password is needed!</p><p>NEW! Just call (800) 941-9260 (Monday – Friday, 8 am – 8 pm CT).</p><p>Important Things to Know  Make sure you understand how much your benefit options will cost you out of your paycheck in 2018. If your costs are not displayed on the website, they will be provided by your employer.  You must enroll by December 20, 2017. If you don’t enroll, you will automatically keep any benefits you are already participating in this year as long as that coverage is still available in 2018. The coverage level will remain the same, but your premium costs may change.  Remember: After Annual Enrollment, you can only change your benefits when you experience a qualified life event, such as marriage, divorce, birth, a change in your job classification, or a change in your or your spouse’s employment status that affects your benefits eligibility. If you should need to cancel any of your benefits during the year, you’ll be able to do that by logging in to the enrollment site through www.MyMcDBenefits.com again. The termination would be effective the last day of the month in which you cancel any coverage.</p><p>Owner/Operators are independent employers and make their own policies regarding employment-related matters. Owner/Operators may choose to use information from these materials that will be helpful to them in operating their own McDonald’s restaurant(s). If you work for an Owner/Operator organization, please check with your Owner/Operator, or the person designated by your Owner/Operator, to determine whether and which of these materials apply to your organization.
