<p> Hart Athletic Department Hall of Fame</p><p>The mission of the Hart Public Schools Athletic Hall of Fame is to recognize individuals who have made exceptional achievements in their respective sport and/or significant contributions to the Hart Athletic Department and the lives of its athletes. Graduation from Hart High School is not a requirement to be inducted into the Hall of Fame. </p><p>Established in 2015, the Hart Public Schools Athletic Hall of Fame is entering its first year of existence. We are extending an invitation to everyone associated with the Hart Athletic Department and Hart Public Schools to submit a nomination. Please see the criteria below:</p><p>Individual Athlete – Athletes will have demonstrated outstanding accomplishments in the field of athletics at the high school, college, or professional levels. Each athlete must be 10 years removed from their high school graduation prior to nomination. </p><p>Coach – Coaches will have achieved significant success as a member of the coaching staff. Each coach must be retired from coaching and have served Hart Public Schools for at least ten years. </p><p>Support Personnel – Individuals nominated will have demonstrated sustained contribution to the Hart Athletic Department and have served as a major influence in the lives of Hart athletes, staff, and programs. These individuals must have at least 10 years of service to the athletic department. </p><p>Team – Teams nominated should have achieved significant success and accolades as a high school athletic team and/or program. Each team should be at least 10 years removed from final year of successful season(s) prior to nomination. </p><p>Each inductee into the Hart High School Hall of Fame will have a plaque including their photograph and a short biography displayed permanently on the ‘Hall of Fame’ wall outside of the gymnasium. New inductees will be chosen by a committee comprised of school and community members, and selected from those nominated. Nominees will be permanently on file and automatically considered each year, therefore, not requiring yearly nominations. A special induction ceremony will be held to honor those selected each year.</p><p>Please complete the nomination form and return to Jason Hernandez (231-873-6228), 300 W.
