<p>HIGH AGENCY PROCEDURES VALUED HOMES PROGRAM</p><p>WHEN QUOTE IS REQUESTED Submit Fully Completed High Valued Homeowners Application to STL/High Valued Homes. On Beach/Coastal risks a copy of the Flood Policy and Elevation Certificate (if one exists) will be necessary. Client “signatures” are required on applications if the quote is accepted. (The application may be submitted by Fax, US Mail or by E-mail)</p><p>Once the producing agent receives the quote from Global Special Risks/Lloyd’s, there will be a period of approximate 21 days to accept the quote, depending on the requested effective date. The quote letter will explain the time/payment requirements.</p><p>WHEN QUOTE IS ACCEPTED BY CLIENT The producing agent is to submit request in writing to have the risk “bound” (E-mail or Fax). Global Special Risks, Inc. will issue a “Confirmation of Cover” and it will be sent to the producing agent. This will describe coverage, premium, deductibles, exclusions, additions, etc. </p><p>PREMIUM PAYMENT An invoice will be provided by STL/HVH and payment will be required within 15 days. Checks are to be made payable to STL/HVH Program and mailed to the attention of Doug Day, 100 Europa Drive, Suite 571, Chapel Hill, NC 27517. </p><p>The NC Form F is to be signed by the client and broker, and if the producing agent has an E&S license, the form is to be completed in total and signed. If the producing agent does not have an E&S license, STL will complete and sign that portion of the Form.</p><p>For SC risks, the required form will be provided.</p><p>NEW POLICY ISSUANCE In approximately 10 days a new policy will be sent to the producing agent.</p><p>CLAIMS All policies include a special claim form to be used in the event of a claim.
