<p> COLLECTIVE IMPACT CHARTER WINDSOR-ESSEX COMPASSIONATE COMMUNITY COALITION </p><p>VISION: Working together to empower citizens in Windsor-Essex to live, thrive and prosper.</p><p>OPPORTUNITY: We are united in our desire to build a community care system that is good enough for our parents, our children, and ourselves as we age. With compassion, we reveal our heart and build our engine, make it easy to collaborate and innovate, and aspire to be better. </p><p>COMMUNITY SPACE: Use community assets to empower citizens. Invest in community culture, social relationships as well as services to raise our collective quality of life. Grow capacity and strengthen community through four (4) new inter-connected, collective action portals: 1. Personal Care Portal: Empowering citizens to live and die well 2. Neighbours Portal: Connecting neighbours who care 3. Outreach Portal: Reaching our most vulnerable; and 4. Leadership Portal: Improving life and living, for all of us </p><p>Care Model: Help aging and vulnerable citizens and care partners receive care that is right for them – care that is personalized, predictive, preventative, participatory, and population-based; joined up across citizens, care partners and community; and lasting a life-time (5P+3C life-long care).</p><p>COLLECTIVE IMPACT: Scattered and fragmented people, programs and relationships achieve isolated impact. Connected people, programs and relationships can achieve a collective impact. By pooling collective community assets and talents, we extend the reach and extract more value from existing services, making it actually possible to care more over a life-time (growing the pie, not just consuming it). Volunteerism and neighbourliness become fully embedded in citizenship, improving the health and happiness of everyone. By working together, we will improve community’s Triple bottom line and demonstrate year over year improvement on shared outcomes that matter to citizens and community – like equity, quality of life and value.</p><p>SCALE UP AND SPREAD: A community’s health tapestry is woven from its social fabric.
