2006 Application Form: First Year Genetic Counseling Student NMH Merit Scholarship

<p>2014 Northwestern Memorial Hospital (NMH) Merit Scholarship</p><p>We are pleased that a portion of funding from NMH to the Center for Genetic Medicine (CGM) is being used for a two merit based scholarships. We hope that these merit- based scholarships can be used to identify candidates who have the potential to become outstanding genetic counselors. </p><p>The scholarship program funds two incoming students during the first year of training, at a level of $3,000/quarter in tuition relief, plus a $500 book allowance (for a total of $12,500 over the first year). </p><p> The following criteria will be taken into account when awarding the scholarships: applicants who are underrepresented (<15%) within the profession of genetic counseling (including African American, Asian and Hispanic backgrounds as well as males), previous academic performance, financial need, and those from rural and underserved areas. </p><p> Candidates will be selected on the basis of an application and one page personal statement (stating reasons for applying for the scholarship and how they fulfill one or more of the above criteria). The awardees will be chosen by the Program Director, Associate Director and Assistant Director.</p><p>General responsibilities of Scholarship Recipients: </p><p> Students who receive the NMH scholarship may also hold scholarships or awards from external organizations. If additional financial support is needed by NMH scholarship recipients, the graduate program will try to assist students in finding reasonable and related part time work or work- study positions. </p><p> Students who receive the NMH scholarship will be required to produce at least one abstract (accepted as a presented paper or poster), paper or case report in conjunction with program faculty during the duration of their funding time.</p><p> Scholarship awardees will be required to provide an executive summary of training yearly to the scholarship administrators.
