NOT in any way be made in order to obtain personal, financial, or commercial gain.</p><p>2 Request solely by external organizers must obtain the approval from the Director of CTLE.</p><p>3 Reservation should be submitted at least 2 working days in advance, and are processed on a first-come-first-serviced basis. Confirmation depends on availability of specific facilities. Immediate booking will not be entertained without justified reason. The reservation must not exceed a maximum of 1 year.</p><p>4 The maximum duration of renting electronic facilities is 3 days, if more time is needed, applicants have to request for approval from the Functional Head of CTLE.</p><p>5 All electronic facilities are expected to return on time, late return will contribute to inhibition of future reservation.</p><p>6 Users are responsible for all matters pertaining to the proper use of the electronic facilities. The cost of repair and / or replacement of any loss / mutilation of the electronic facilities will be borne by the users.</p><p>7 CTLE will assist users in the use of the electronic facilities, ONLY within his/her scope of knowledge. CTLE has no formal teaching responsibility.</p><p>8 Unless otherwise agreed, a user's right to use the electronic facilities is not transferable and is subject to any and all limits or restrictions established by CTLE.</p><p>9 In the event of any disagreement over the above mentioned policy, terms, and / or conditions, CTLE’s decision / interpretation shall always prevail.</p><p>I understand and accept the above statements.</p><p>______Signature Date</p>
