<p> Purple Heart Questions</p><p>I offer the following questions in order to get details for your Purple Heart Incident. I will follow up with any questions for you that I may have. Thanks for participating in this endeavor.</p><p>Question 1. Your name, rank, MOS and unit at time of incident.</p><p>Question 2. Provide the date of the incident.</p><p>Question 3. Who was your platoon leader, squad/fire team leader? Name as many members from your unit as you can recall, i.e. the medic, etc.</p><p>Question 4. Describe your unit’s mission that day the best you can. (ambush, search and destroy, in night defensive position, etc.).</p><p>Question 5. Begin with the day’s mission. Describe how you and your unit arrived at the incident in which you were wounded.</p><p>Question 6. Was anyone else wounded or KIA in the incident? Who?</p><p>Question 7. Describe the incident itself and how long it lasted.</p><p>Question 8. Describe what happened from the time you received first-aid to arrival at rear aid-station.</p><p>Question 9. Name (your best recollection) of the stops you made from the time you were “dust-off” from the field to the last medical facility in country, Japan, or U.S.</p><p>Question 10. Are you on disability and if so, what percent?</p><p>Question 11. Were you medical discharged?</p><p>Question 11. Give any additional comments, etc. </p><p>Trails Taken After Vietnam</p><p>1. Your Hometown when entering the military? 2. Were you drafted or did you enlist? 3. What did you do after leaving the military service (trails taken)? 4. Are you on any disability now from your wound(s)? 5. Were you married during your Vietnam tour? 6. Where did you meat your wife (first if applies) and when? 7. When did you get married? 8. How many children do you have? 9. How many grand children do you have? 10. Where do you live now? 11. What hobbies do you enjoy most now? 12. What is your contact information? 13. Add whatever additional info. you desire, and don't worry about grammar, spelling, etc. 14. BE SURE TO SEND ME A PHOTO OF YOU IN VIETNAM AND ONE MORE RECENT.
