<p> Can I have an AIDS test? </p><p>Consider: </p><p>Who is requesting?</p><p>The patient.</p><p>The patient's partner or family.</p><p>An insurance company.</p><p>A doctor or other healthcare professional.</p><p>Employer.</p><p>Why are they requesting?</p><p>For reassurance of a negative result.</p><p>For reassurance of a partner.</p><p>For insurance purposes - the risk assessment.</p><p>For pre-employment / work visa requirements (e. g. some Arab states).</p><p>Because of high-risk lifestyle: are their fears valid?</p><p>Because of clinical suspicion.</p><p>What are the implications?</p><p>Of having the test: risk of disclosure to others (unintended or requested)</p><p>- doctors, care workers, insurance companies, family.</p><p>Of a negative result: will lifestyle change?</p><p>Of a positive result: counselling and support and referral.</p><p>Family / work / social life.</p><p>Health.</p><p>Early intervention in treating opportunistic infection.</p><p>Who else needs to know - partner, children, health workers.</p><p>- can they be informed without consent? Where should it be done?</p><p>In the surgery - part of "holistic" care.</p><p>In the G U clinic - no record in GP notes (Improved confidentiality or impaired care?)</p><p>When should it be done?</p><p>Three months after risk exposure.</p><p>Other considerations:</p><p>Counselling for having the test is separate from performing the test and accepting responsibility for informing the patient of the result.</p><p>Do you have the skills and access to support and follow up to break the news of a positive result?</p><p>What should we write in the GP notes?</p><p>Can we use the discussion as an opportunity for lifestyle advice - the patient may decide not to proceed and see nobody further?</p><p>What does the patient know of HIV and Aids?</p><p>Confidentiality - GMC guidelines and booklet.</p><p>Ref.
