Contact your insurance carrier to be sure that you are eligible for our services, also if you are required to obtain a referral. If your insurance carrier requires you to have a referral, you must obtain one from your primary care physician PRIOR to your appointment. 2. Please arrange to have a copy of your medical records transferred from your previous physician. We have a medical release form included in this package for your convenience. At your first appointment, please bring with you: 1. Your insurance card(s) 2. Driver’s License or other photo ID 3. Any required co-payment 4. A medication list, or the bottles of medication you are currently taking</p><p>We are located in the "210 Building" Homeport Office Complex on Jones Road, at intersection of Jones Road & Gifford Street. This is also directly across Gifford Street from the Coonamessett Inn. We are located on the ground floor to the right.</p><p>From Route 28 and points north, travel on 28 South to traffic light at Falmouth Hospital; turn LEFT onto Jones Road; proceed almost to next traffic light at the intersection of Jones Road and Gifford Street, turn LEFT into Homeport Complex (opposite Nursery Road), just beyond the "210 Building"; we are accessed through the main entry to your left, where parking is located.</p><p>From the intersection of Brick Kiln Road and Gifford Street at the Falmouth High School traffic light, travel south on Gifford Street, past the Falmouth D.P.W. and Lawrence Lynch Contractors, and proceed almost to the traffic light at the intersection of Jones Road and Gifford Street; turn RIGHT into the Homeport Office Complex, just before the bank and traffic light; at STOP sign, turn LEFT, and our building (210 Building) will be directly in from of you to the right.</p>
