<p> MEMBERSHIP APPLICATION INSTRUCTIONS</p><p>Thank you for your interest in the American Society of Shoulder and Elbow Therapists. The application form is designed for you to type directly onto it. To do this, you will need to have Adobe Reader ® installed on your computer. If you do not have Adobe Reader® on your computer you may download a free version from http://www.adobe.com. Alternatively, you may print the application form and complete by hand. All applications must be signed and submitted by mail to the executive secretary at the address below. </p><p>You are required to submit two letters of recommendation with your application. One letter from either a physical therapist, occupational therapist, or athletic trainer; and the other from an orthopaedic surgeon who is familiar with your work in the area of shoulder and /or elbow rehabilitation. The letters of recommendation should be separate documents, on professional letterhead, with the author’s name, address, telephone number and email address. The letters of recommendation should be submitted with the applicant’s packet in their own sealed envelope.</p><p>Applications will not be processed unless ALL of the following are included in a single application packet.</p><p>$25 application fee (non-refundable) Signed completed application form Statement of Interest Current Curriculum Vitae Copy of License Letter of recommendation from either an occupational therapist, physical therapist, or athletic trainer in a sealed envelope.
