<p>TOP TIPS GUIDANCE FOR HEAD & NECK 2 WW</p><p>REFER: DYSPHAGIA which is progressive  with oropharyngeal symptoms– refer to ENT (2 ww)  with retrosternal symptoms – refer upper GI (2 ww) PERSISTENT HOARSENESS for 3 weeks URGENT chest X-ray  Positive CXR refer to chest physician (2ww)  Negative CXR refer to ENT (2ww) UNILATERAL OTALGIA with normal otoscopy TRACHEAL COMPRESSION symptoms including stridor due to thyroid swelling UNEXPLAINED :-  neck lump persisting > 3 weeks  ulcers in mouth >3 weeks  red/white particles in mouth + pain, swollen and bleeding  persistent unexplained sore throat  tooth mobility for >3 weeks (refer urgently to dentist)</p><p>THYROID Thyroid nodules are common Thyroid cancers are rare (increased if male/elderly) LUMPS: Test TSH Urgent Referral for:  solitary nodule increasing in size  history of neck irradiation  family history of endocrine tumour  unexplained hoarseness or voice changes  cervical lymphadenopathy  pre-pubertal patients or > 65 Years</p><p>(Thyroid/neck lump clinic Wed am most can have an ultrasound that morning)</p><p>NB: 80% of 2ww referrals are Laryngopharyngeal reflux:- Consider if: FLUCTUATING symptoms Lump in the throat Throat clearing Non-productive cough or spasms of coughing (reflux events) Laryngospasm (often nocturnal) Teachers and call centre workers May be obese Treatment: ↓ Caffeine ↓ Fizzy drinks ↓ Alcohol ↓ Fatty foods Eat regular meals 1st line: Barrier medication: Alginate raft-forming preparation after every meal & at night 2nd line: PPI if remain symptomatic > 2 months. BD 30 mins < breakfast and 30 mins < evening meal.
