<p> ROCKETS FOR SCHOOLS GREAT LAKES SPACEPORT EDUCATION FOUNDATION, INC. Spaceport Sheboygan</p><p>COVENANT NOT TO SUE AND INDEMNITY AGREEMENT</p><p>I. Agreement</p><p>I will voluntarily participate in the Rockets for Schools Program held the third week of May in Sheboygan, WI. This event is sponsored by Great Lakes Spaceport Education Foundation, Inc, Sheboygan Area School District, Space Port Florida Authority, and the City of Sheboygan.</p><p>I agree that the above named sponsoring parties have taken all reasonable precautions necessary to protect me from the known danger inherent in launching rockets; however, I also acknowledge that I may be exposed to other unknown hazards associated with this and other operations at the launch site in Sheboygan.</p><p>In consideration for the above named sponsors permitting me to participate in the tour and rocket launch activities at the City of Sheboygan launch site on the west end of the South Pier and the Armory in Sheboygan, I, for myself, my heirs, administrator, executors, and assignees, hereby covenant and agree that I will never institute, prosecute, or in any way aid in the institution of prosecution of, any demand, claim, or suit against the above named sponsors and or their officers, agents, employees, or facility users, acting officially or otherwise, for any loss, damage, or injury (including death ) to my person or my property which may occur from any cause whatsoever as a result of my participation in this activity.</p><p>If I should demand, claim, sue, or aid in any way in such a demand, claim or suit, I agree to indemnify the Great Lakes Spaceport Education Foundation, Inc./Rockets For Schools Program and its sponsors the Sheboygan Area School District, and the City of Sheboygan, for all damages, expenses, and costs they may incur as a result thereof. I understand and agree that I am assuming the risk or any personal injury, death, or property damage that may result while participating in this activity, including those that may be caused by the negligence of the above named sponsors.</p><p>The term sponsors as used herein includes the Wisconsin Department of Transportation , the Wisconsin Air Guard, the Coast Guard, the Sheboygan Area School District and the City of Sheboygan, Wisconsin, their officers, agents, contractors, employees or any persons using facilities in conjunction with the launch of the Super Loki Rocket in Sheboygan, Wisconsin acting officially or otherwise.</p><p>Participant's Signature ______Date: ______</p><p>II.
