
<p> St.Johns SIRS and Sepsis Alert</p><p>RN responsibility:</p><p> Check Activity column (Nurse M tab) for Patient Care tasks </p><p> This is what your SIRS or Sepsis Alert will look like: </p><p> It will be present under Encounters column if there is patient data</p><p> indicating SIRS or Sepsis. The blue “S” will display if the nurse completes the screening form and the patient has a positive SIRS or Sepsis screen.</p><p> When you see the go into the activities column and double click on the green cross to see</p><p>Patient Care tasks . The task will tell you what criteria the patient has that created the task (see red underline in picture below). </p><p> Check the box in front of the task and click document to open and complete the Sepsis Screen form. St.Johns SIRS and Sepsis Alert</p><p> Some alerts may trigger from competing diagnoses (e.g., asthma, afib). You should still complete the sepsis screen and open a focus note from the form to indicate that alert not appropriate for patient.</p>
