The information provided should encompass your high school career from the beginning of your freshman year to the present.</p><p>Personal information: Provide your name, mailing address, email address, phone(s), gender, date of birth, place of birth, name of high school(s), parents’ names and marital status, siblings names and ages, and denote any siblings presently attending college.</p><p>Scholastic: Provide an official school transcript and SAT or ACT results.</p><p>Work experience: Provide the specific nature of work, your employer(s), dates, and hours worked.</p><p>Community Service: Provide the specific nature of service, the organization(s), dates, and hours worked.</p><p>Honors and Awards: What honors and awards show off your achievements (such as Eagle Scout, Boy’s/Girl’s State, Teen of the Year, HOBY, major athletic and/or academic awards)?</p><p>Leadership roles: What offices have you held in or out of school (president, secretary, treasurer, team captain, etc.)? Briefly describe your responsibilities.</p><p>Extracurricular Activities: What is your involvement in athletics, clubs, and organizations?</p><p>Essay: (one page or less) What are your higher educational and career goals?</p><p>Financial need statement: List applications for other scholarships. Have your parents prepare and sign a statement briefly describing financial assets (gross wages) and needs.</p><p>Exhibits: (optional) You may include copies of certificates, awards, articles showing your achievements and involvement. (Be conservative – more is not better.)</p><p>Applications must be received at the John Day Elks Lodge #1824, 140 NE Dayton Street, John Day, Oregon 97845 on or before May 6, 2016. The Lodge is open after 4:00 PM, Monday through Saturday.
