The yellow highlighted sections need to be completed. </p><p>QITC Module Order Form 4 – Systems Integration – Issued August 2017 - Version 1.0.0 Module 4 – Systems Integration page 2 of 5</p><p>Module Order Form 4 – Systems Integration The following Module Order Form to be completed if the Customer is procuring Systems Integration (refer to clause 3 of Module 4). 1. Commencement of Integration Services Clause 3.1 of Module 4 <<Specify the start date of the Integration Services.>></p><p>2. Integration Services Clause 3.2 of Module 4 <<Specify the Integration Services the Supplier must supply.>></p><p>3. System <<Specify the System the Supplier must provide after it has performed the Integration Services, with reference to all applicable Hardware, Software and other components that comprise the System.>></p><p>4. Requirements Specifications for Integration Services (or annex specifications to the Module Order Form) <<Insert or annex a detailed description of the functional, operational and technical specifications for the Integration Services.>></p><p>5. Implementation planning Clause 3.4 of Module 4 <<Clause 3.4 provides that if specified in the Module Order Form, the Supplier must conduct implementation planning in order to confirm the Supplier’s assessment of the scope and complexity of the Integration Services and the Integration Services required. Specify if the Supplier is required to deliver an implementation planning study and the requirements for the implementation planning study.>></p><p>6. Detailed Specifications Clause 3.5 of Module 4 Documents comprising the Detailed Specifications <<Clause 3.5 provides that the Supplier must prepare drafts of the project documents set out in clause 3.5(a) unless otherwise specified in the Module Order Form. Specify if there are any additional project documents required to be included in the Detailed Specifications, or whether any of the documents set out in clause 3.5(a) are not required to be provided.>></p><p>7. Data Tools Clauses 3.11 of Module 4 <<Clause 3.11 provides that if specified in the Module Order Form, the Supplier must perform the Data Migration Services and Data Cleaning Services using software tools, object libraries, methodologies or other devices owned by the Supplier or any other party (Data Tools). Specify the Data Tools to be provided by the Supplier.>></p><p>12. Ancillary services Clause 3.13 of Module 4 <<Insert details of any ancillary services to be provided by the Supplier (if any) in relation to the Integration Services.>></p><p>13. Warranty Period Clause 3.14 of Module 4 <<Insert Warranty Period for the System. The Warranty Period is defined to commence on the AAD of the System, unless otherwise specified in the Order Documents.
