<p> 5th Grade Classroom Discipline Plan</p><p>EXPECTATIONS</p><p>1. I have the right to a quiet, distraction-free environment. I have the responsibility to control my talking and not distract others.</p><p>2. I have the right to a safe environment. I have the responsibility to keep my hands, feet, etc. to myself.</p><p>3. I have the right to receive directions. I have the responsibility to listen carefully and follow directions the first time they are given.</p><p>4. I have the right to be successful. I have the responsibility to be prepared with my work and materials needed for class.</p><p>5. I have the right to have myself and my property respected. I have the responsibility to respect myself, others, and their property.</p><p>A Responsibility Folder will be kept recording appropriate and inappropriate behavior. Students who behave appropriately will quickly accumulate 25 points in their folder and be positively rewarded. In the case a child should lose 25 points in their folder for inappropriate behavior or failing to complete assignments or homework, consequences will follow. Any severe disruption will be handled through the office and you will be notified.</p><p>It is in the best interest of your child that we work together in all aspects of his or her education. We will keep you informed about your child’s progress in this area. We have already discussed this plan with your child. Please review it together, sign and return the form provided.</p><p>Thank you for your support.</p><p>Fifth Grade Teachers REWARDS</p><p>DURING EACH NINE WEEK PERIOD</p><p>*For every 25 points earned the student will receive a sticker.</p><p>*At every 100 points, students will receive a homework/back to class pass.</p><p>*Any student receiving 10 or less minus points for the entire nine week period will receive a drink (ex. Boxed fruit drink or Gatorade).</p><p>*The boy and girl from each class with the most total plus points at the end of the nine week period will receive a special reward.</p><p>*Any student receiving less than 80 minus points during the nine week period is eligible for an end of the nine week’s incentive. </p><p>INCENTIVES</p><p>1st nine weeks Movie 2nd nine weeks Game Time 3rd nine weeks Choice 4th nine weeks Mystery!!
