<p> What is a Help Me Grow Service Coordinator?</p><p>A Help Me Grow Service Coordinator is a person who works with you to find services to meet your child and family’s needs. Your Service Coordinator helps identify your family’s strengths and needs, find resources, think about decisions, and coordinate all the services being received. Your Service Coordinator also makes sure the rights of your children and family are protected in the Help Me Grow system.</p><p>Your Help Me Grow Service Coordinator: ______</p><p>The person serving as your Service Coordinator can change if your child’s and family’s needs and desires change.
