<p> Association of Montana Troopers</p><p>Application for Member Child Scholarship</p><p>I, ______, legal son or daughter of current AMT member in good standing ______, do hereby make application for a $500 scholarship from the Association of Montana Troopers. I understand that this scholarship is to be used only for tuition and fees associated with my education during the 20_____ and 20_____ school year and that this scholarship is limited to a total or maximum of four years of secondary education.</p><p>I understand that in order for this scholarship to be approved and processed, I must supply the following information in this prescribed manner:</p><p>1. First year secondary education students must enclose a copy of their high school transcript and a letter of acceptance to a higher education institution with this application.</p><p>2. Second or subsequent year secondary education students must submit a copy of their previous year’s transcript(s) with this application. (Successful applicants may not be on academic probation and must be making progress towards a degree.)</p><p>3. This fully completed application and item(s) required under numbers one or two must be submitted by e- mail to scholarship chairperson Bob Hensley at: [email protected] by the deadline of March 15th of each year. (Please scan and attach the fully completed application and accompanying documents as necessary.)</p><p>4. Also by March 15th of each year, all applicants must submit a current JPEG digital photo and a separate, short, electronic (Word based) biography suitable for publication directly to Montana Trooper magazine editor Kristin Banchero at: [email protected]. (Please adhere to the separate JPEG photo and Word based formats.
