Purpose of the Form

<p> Instructions for Completing the OIT Request for Services Form</p><p>Purpose of the Form The OIT Request for Services Form is to be used to request application installations, programming services, server installations, system upgrades, mod tape installations, or training development from OIT. To request personal computer services or telecommunication/networking services, use the Service Request Forms specific for those areas. Request # When OIT enters the Request for Services into the Project Management System, the (office use only) System automatically assigns a Service Request Number. This number is used throughout the life of the project to record all work processes and time required for the completion of the project. Requestor Name and department of the individual making the request. The Requestor is designated as the primary contact for questions, details, or client requirements. Phone # Requestor’s phone number. Fiscal Account # Requesting department’s FAS account number. Dated Submitted Enter the date in mm/dd/yyyy format. Example: 07/01/2003. Date Needed Do not use ASAP. Enter the date in mm/dd/yyyy format. Example: 07/01/2003. Requestor Priority Requestor enters priority based on the perception of the need of the request. Type of Request All requests submitted by clients are Capital requests and have specific beginning and (Capital or ending dates with identifiable requirements that can cross fiscal year lines. Requests Expense) generated internally by OIT may be either Capital or Expense requests. Expense requests are used only when the project is designed to track time posted to specific maintenance projects that start at the beginning of the fiscal year and end at the close of the fiscal year. Fiscal Officer All requests must be signed by a fiscal officer and submitted to OIT before any work Authorization can begin on the requested project. Internal OIT requests require the manager’s signature. Project Management OIT staff complete the description when the project is entered into the Project Description Management System. Description of The Requestor will need to provide specific information and details of the request. Request Impact for This is to be completed by the requestor and should be as specific as possible including Department and cost and labor savings that will occur if this request is completed by OIT: why should University it be done and what are the advantages. Some examples are:</p><p> Modify to comply with changing business requirements such as . . .  Comply with new or changed government requirements.  Correct an application defect since the application is not performing as designed.  Improve the processing of the application or modify it to reduce operating or other costs by . . .</p><p>If more space is required, please attach additional sheets with extended description or cost saving.</p><p>To Save Data Click on the save data button at the bottom of the form.
