<p>Meal Delivery Procedure: meals served on site in a different location from cafeteria</p><p> Meal Delivery form: required when student(s) is not present to select their meals o Manager/ Satellite Cashier will take all trays to the classroom to ensure procedure is followed. o Follow directions on Meal Delivery form o Student is allowed offered vs. serve. If an item is refused, the food item must be thrown away when returning to the kitchen o Manager/SC checks for any special diet menus prior to trays leaving the cafeteria. . Special diets must be prepared per special diet menu o Mgr/SC- provide an electronic copy of the Meal Delivery form to scheduled deliveries (ex: if SAC receives a delivery daily, email the form to the teacher for her to send to you daily with names typed in) . Request that you receive the Meal Delivery Form in a timely manner, so you have time to prepare the trays. . Work with your teacher/staff member & have designated times when the trays will be delivered (minimizing trips to the classroom).  Feeding Location: o Menu posted o HACCP/ Civil Rights sign posted o Water Availability sign o Serving gloves provided o Water pitcher & cups provided (only if a water fountain is not assessable during meal service)</p><p>Rev. 01/09/15</p><p>FIELD TRIP Pick Up form:</p><p>To ensure that TDA procedures are followed, the following procedure for FIELD TRIPS must be used: </p><p> Teacher must inform cafeteria manager at lease two weeks in advance of a field trip. Provide date, time and how many meals will be needed. All food must be kept at the proper “safe” temperatures. Ice chests will be needed. For Field trips only peanut butter & jelly or cheese sandwiches is offered. o Managers must check student list for special diets. o Teacher cannot be given a child’s meal status, because this is confidential information (overt identification). </p><p>Field Trip Pick-up Form:</p><p> Teacher – Follow directions on the Field Trip Pick Up form & returns form to cafeteria manager prior to field trip  Manager will fill in amount owed & check if a student is on a special diet (this student will receive a menu specific to his/her special diet) o Manager must make a copy and return to teacher (this way the teacher knows which student owes money).  Teacher- Day of field trip: o Verify/update students on form . Cross out names of students not in attendance or not participating in the field trip. Teacher must make a copy of the updated form for cafeteria manager along with amount collected, prior to picking up meals. o Teacher on field trip will fill out field trip form during distribution of meal . Student will sign when he/she gets a meal (if student is unable to sign for meal, teacher may sign for them) . Teacher will check the box – at the time the student receives full meal . Teacher will return the Field Trip Pick up form to cafeteria manager upon returning from the field trip.  Manager-Day of field trip: o If students arrive back before Primero is closed, manager will give the field trip pick up form to cashier to enter students in the POS. o If students arrive after primero is closed, manager will call federal meals and inform them the field trip has not been entered in Primero. Manager will check box on the field trip form that indicates that federal meals is to ring them up. o If meals are brought back after the field trip, then food must be discarded.</p><p>Rev. 01/09/15</p>
