<p>Pennsylvania Humanities Council Evaluating Commonwealth Speakers: Instructions for Presenters</p><p>Introduction As of April 2015, speakers should complete their online evaluations in SurveyMonkey using the link provided below. Evaluations are part of a performance measurement system that will increase the capacity of the PHC to gauge the impact and effectiveness of our programs. </p><p>If you have questions or comments, please contact PHC at (800) 462-0442 or (215) 925-1005 or [email protected]. </p><p>Evaluating a PHC Program There are two components to evaluations: </p><p>1. Complete the online SurveyMonkey evaluation form here: https://www.surveymonkey.com/s/CWSPRESENTER.</p><p>NOTE: No username or password is needed to access the system. </p><p>If you can’t access the form by clicking on the link, copy and paste the link directly into your browser.</p><p>2. Submit copies of your travel expense report and receipts by email, fax or mail. Mailing address is Pennsylvania Humanities Council, 325 Chestnut Street, Suite 715, Philadelphia, PA 19106-2607, attention: Commonwealth Speakers Program.</p><p>Both components must be completed within two weeks of your presentation. Note: Your honorarium will be issued upon completion of the evaluation.</p>
