Failure to submit these reports on a weekly basis may result in the withholding of payments to the Contractor.</p><p>Complete the project identification information, name of prime contractor or subcontractor, report number starting from one and numbering consecutively, and the week-ending date.</p><p>Select a Training Program: FHWA – $0.80 per hour reimbursement for training special provision administered in accordance with 23 CFR Part 230. IDOT MRB - $3.50 per hour reimbursement for training special provision on select IDOT District 8 projects. TPG - $10.00 per hour reimbursement for training given certified graduates of IDOT’s pre-apprenticeship Highway Construction Careers Training Program (HCCTP).</p><p>Column 1 - Trainee Name and Individual Identification Number: The trainee’s full name and individual identification number (e.g., the last four digits of the employee’s social security number) must be shown on each weekly report submitted.</p><p>Column 2 - Ethnic Group: Indicate trainee’s ethnic group using the following legend: B-Black, H-Hispanic/Latino, I-American Indian/Alaskan Native, A-Asian, P- Pacific Islander/Native Hawaiian, C- Caucasian, O-Other.</p><p>Column 3, 4, and 5 – Training Program: Indicate applicable training program.</p><p>Column 6 - Work Classification: List classification descriptive of work actually performed by trainees. Also indicate a two-letter code for each work classification using the following legend:</p><p>EO - Equipment Operators EL - Electricians IW - Ironworkers PP - Pipefitters, Plumbers CA - Carpenters PA - Painters CM - Cement Masons LA - Laborers</p><p>Column 7 - Status: Indicate whether trainee is enrolled in an apprenticeship program (A) or an on-the-job training program (T).</p><p>Column 8 - Hours and Days Worked: Indicate the day and the number of hours worked each day.</p><p>Column 9 - Hours This Week: Total the hours for the week for each trainee.</p><p>Column 10 - Hours to Date: Indicate the accumulative total hours worked by each trainee on the project to date including the present week.</p><p>For information about IDOTs collection and use of confidential information review the department’s Identity Protection Policy.</p><p>Printed 4/4/2018 SBE 1014 (Rev. 01/28/15)</p>
