<p>Revised 03/13</p><p>Horizon Kids School-Age Care Program Parent Packet Please retain this information for future reference Policy Information</p><p>Registration Specifications A $30.00 non-refundable registration fee per child is required when enrolling The following forms must be completed and turned in prior to attending: * Personal Data Sheet * Health History Form * Authorization for Emergency Medical Care Form (Notary Required) * CACFP Enrollment Form * Long-Term/Short-Term/Self-Administer Medications Form (If applicable)</p><p>Horizon Kids Administration The Leavenworth Public Schools Education Foundation will operate the before and after school program. We will adhere to school districts policy on education and disciplinary actions. Our program will not discriminate on the basis of race, color, national origin, sex, or religion.</p><p>Program Fees Fees will be automatically deducted from your account on a bi-weekly basis. Returns on account transfers will result in a $30.00 charge and termination of care until the situation is resolved. Fees will not be prorated due to holidays, shortened school weeks, student illness, behavior issues/termination, withdraws, student vacations, etc.</p><p>Withdrawing from the program The Horizon Kids program requires a two-week written notification prior to withdrawing from the program. No refunds will be given. This policy enables an easy transition out of our program. </p><p>After hours charges If you are late picking your child up after 6:00pm, a $1.00 a minute will be collected that night when your child is picked up. Your child will not be able to return to the program until after hours charges are paid in full. Continued lateness could result in termination from the program. If you suspect you will be late, please give us a call. </p><p>Authorized Pick-up Individuals authorized to pick up your child must be listed on your registration form. Authorized individuals may also be added throughout the school year. The Horizon Kids program recommends those authorized to pick up to be at least 16 years of age with a valid ID. If an authorized individual is younger than 16 and is walking the child home, we require a KDHE form(CCL-034) completed and on file. For safety purposes we require authorized individuals to provide Horizon Kids staff with a photo ID when picking up your child. If parents and/or authorized individuals appear to be impaired by drugs or alcohol, state law prohibits the release of the child. A copy of existing court orders excluding parent contact, visitation and child release must be on file, on site to sustain law enforcement involvement. Playground The Horizon Kids program uses the playground during our hours of operation. A health department regulation requires only Horizon Kids patrons currently signed in for that day by Horizon Kids staff to be authorized on the playground during our outdoor activities. This policy is stated in the Leavenworth Public Schools Student Handbook. All non-Horizon Kids patrons and signed out Horizon Kids patrons would be deterred from being on the playground at that time. </p><p>Illness/Communicable Diseases Children with the following symptoms: (vomiting, diarrhea, and /or fever) will be sent home and not return until symptom free for 24 hours. Other conditions such as injuries, rashes, pink eye, etc., would require your child to be picked up. Students should not be in attendance if they are suffering from any communicable disease. We will follow those guidelines determined by the Leavenworth County Health Department. </p><p>“Open-Door” Policy We encourage you to visit or just drop by to see your child in our program. If you have any special talents or skills that would serve as an educational activity, contact the site director.</p><p>Dress Code To foster a pleasant environment afterschool, we will follow the school dress code. Students are expected to dress appropriately for afterschool activities and weather. Students are expected to be neatly and cleanly dressed at all times. Students may not wear T-shirts, jackets, and other articles of clothing, which have slogans, lettering, pictures, or graphics, which advertise or promote the use of tobacco, alcohol, sex, violence, or illegal substances or contain profanity. Students may not wear clothing, emblems, badges, symbols, or colors that promote gang affiliation or activity. No halter tops, midriffs, or half-shirts. No bagging or sagging of pants. No hats or bandanas unless permitted by the director. Tennis shoes are recommended because of the gym and outdoor activities. No face painting or hair coloring unless permitted by the director. Age appropriate swim attire during the summer camp.</p><p>Prescription and Non-Prescription Medication If your child will be taking any prescription or non-prescription medication during our program hours, please fill out a medication form. All prescription and non-prescription medication must be in its original container, must have RX information affixed and contain the daily dosage. </p><p>Personal Belongings Personal belongings such as toys, radio, CD player, video game, etc. are prohibited. If personal items are needed on a certain day, the site director will inform you. Those items should be labeled with your child’s name. Horizon Kids is not responsible for loss, theft or breakage of personal belongings.</p><p>Field Trips and Transportation Children will be transported by a leased bus service for all field trips. All children attending on a field trip day will be required to attend the trip. If you do not want your child to attend a field trip, you will need to make arrangements for your child to be picked up prior to the group’s departure. Daily Attendance and Absence Procedures Those days you have selected for your child to participate in our program must stay consistent each week. </p><p>We require the parent dropping off your child for the before school program or an extended care day, to physically walk your child in the building and sign your child into the program. You will place your initials and current time in the available space across from your child’s name for that day. This policy reinforces your child’s safety.</p><p>Horizon Kids staff will be checking each child into the program when they arrive in the afternoon. When you are picking up your child in the evening, we require you to sign your child out. This policy reinforces your child’s safety.</p><p>If your child is absent from school, parents are required to call Horizon Kids to inform us that your child will not be attending our program. If your child does not show up to our program on his/her selected day(s) and we do not receive a call from the parent, we will collaborate with the school office in regards to absences and we will make phone calls to parents to ensure your child’s safety.</p><p>Early Release Days 2013-2014 There will be 14 early release days built into the calendar for professional development opportunities for staff. The Horizon Kids program will provide care from dismissal til 6pm for those currently enrolled in the before and after school program at the following status level: Full time before and after school participants will not be charged an additional fee for the early release days Full time after school participants will not be charged an additional fee for the early release days Part time after school participants—if Wednesday is a regularly scheduled day to attend, you will not be charged an additional fee for early release days</p><p>All other enrolled participants and those outside patrons that will be taking advantage of the “Horizon Kids Early Release Day Package” will be in the programs care on early release days from 2-3:30pm.</p><p>Communication You will receive information regarding our program through: dry erase bulletin boards, newsletters, e-mails, phone calls and notes home. We encourage parent feedback regarding your child and our program. </p><p>Behavior Behavior is very important before and after school. If a child’s behavior is unacceptable, the child behavior will be documented. Depending upon the severity of the incident, a call may be made to the parent to come pick up his/her child.(This includes on field trips) After 3 documentations, the next incident will result in a suspension from the program for up to 3 days. If an incident occurs when the child returns from suspension, the child will be terminated from the program. Leavenworth Public Schools Education Foundation has the right to determine suspension and termination based on the severity of the incidents. Fee refunds will not be given if the Horizon Kids program terminates a child from the program. Termination from either the fall or summer program may hinder your child’s ability to participate in our following fall or summer program. Snack/Meals The before school program will provide your child the opportunity to eat breakfast through the school food service program. If you decide to have your child eat breakfast, you will need to contact the school office to set up your meal ticket. After school snack will be provided in the PM following school dismissal. Snack is provided through the Leavenworth Public Schools Child Nutrition Center. On days when school is not in session, your child will need to bring a sack lunch and drink. Kitchen services will not be available on these days. We will provide a morning and afternoon snack for your child.</p><p>CACFP The Horizon Kids program participates in a federally funded Child and Adult Care Food Program (CACFP). This program ensures our meals and snacks follow federal regulations to provide healthy choices for meals and snacks. Due to CACFP regulations, children may not bring their own breakfast or snacks. </p><p>Snow Days Our program will operate on days the schools are out because of snow or inclement weather. All sites will combine. Location(s) TBA. Our hours on snow days will be 7:30am-6:00pm. Starting an hour later allows our staff to arrive to work safely. Remember to have your child bring a sack lunch and drink. </p><p>On some occasions our program may close due to severe ice or severe weather, so it is important to watch KMBC-TV9 or KCTV5. If we close we will be listed as “Leavenworth Horizon Kids”. If it is not posted, our program will be in session. </p><p>Winter Break The Horizon Kids program will not be providing care during 2013-2014 winter break, Dec. 23-Jan. 1, 2014. The program will operate on January 2 and January 3 if a minimum of 15 students are signed up. </p><p>Spring Break In order for the Horizon Kids program to operate during spring break, the program requires a minimum of 15 children to be signed up each of the break days. Our hours of operation would be 6:30am-6:00pm. A morning snack and afternoon snack will be provided. Your child would need to bring a lunch and drink in an insulated container. All sites would combine at one site location.(TBA) </p><p>Emergency Closure Emergency situations such as utility outages, gas leaks, natural disasters, etc. can result in the closure of our program. If such a situation occurs, the site director will contact you. Utility outages and emergency evacuations of students during the regular school day will result in the afterschool program being cancelled and will reopen after regularly scheduled classes have resumed. </p><p>Confidentiality Statement The following contents of your child’s file are confidential documents: registration form, health history, emergency release, medication form, correspondences from parents and staff, IEP reports and incident reports, and any additional forms necessary. Your child’s file may not be duplicated, distributed, discussed or viewed by anyone other than the custodial parent/guardian, Horizon Kids staff, law enforcement, KDHE designee and emergency medical personnel. The custodial parent/guardian reserves the right to make their child’s records available to designated parties through signed permission. The custodial parent/guardian or SRS child protection agency may obtain the child’s records. A court order must be mandated for the release of child’s files content under all other circumstances. Your child’s records will remain on file at the program site throughout your child’s attendance. If your child is a full-time before and after school participant, there will be NO ADDITIONAL charge for early dismissal, teacher in-service, teacher conference and snow days. A $20.00 charge will apply to those that attend on the above days that are NOT full-time before and after school participants. **SNOW DAYS** HOURS WILL BE 7:30AM TO 6:00PM ALL SITES WILL COMBINE. Spring Break (Separate fee rate) A minimum of 15 children each day is required for the program to operate during break(s). If the minimum is reached, care will be provided from 6:30AM-6:00PM. ALL SITES WILL COMBINE. LOCATION(S) TBA</p><p>Break is not charged at your regular fee rate. If you would like to participate in care during the break the cost to everyone is $20.00 per day, per child.(no 2nd child discount) Forms will be available through your site director prior to the breaks. Current participants will be given first priority and then if room is still available, forms will be available to outside participants. 2013-2014 Fee Payment Schedule Fees will be transferred bi-weekly (Fees are grouped in two-week increments. You will actually be paying your fees two weeks prior to service.)
