<p> Hazardous Materials Shipping Program http://ehs.psu.edu/hazardous-materials-shipping/overview EHS Contact: Barry Cowher at [email protected] 814-865-6391</p><p>Who does this program apply to?  University personnel who ship hazardous materials. o This always includes: . Any shipment containing Dry Ice . Flammable Liquids . Infectious Materials . Biological Specimens . Compressed Gas Cylinders . Other hazardous materials What are the key requirements of this program?  Work units must: o Ensure that individuals shipping hazardous materials, off-site, are aware of and comply with SY 34, Hazardous Materials Shipping  Shipper must: o Provide EHS with: . Name, full address, telephone number, and email address for both sender and recipient. . Detailed description of material to be shipped. . Sender or recipient’s account number for shipping company, i.e. FedEx or UPS. o Provide EHS with approval documentation from Export Control Office for all international shipments. Contact export control officer at [email protected] for approval. o Sign Shipper’s Declaration to certify material is what they say it is. o Provide Dry Ice and container for shipments as required. o Deliver package to an on-campus FedEx pickup location. o Ensure shipment has been picked up by FedEx.  EHS will provide the following at no charge to the shipper: o Consultation on materials to determine if they are hazardous materials o All necessary paperwork for regulated shipments o Proper packaging materials and labels o Schedule the pickup with shipping company o Provide tracking number for the shipment and email updates on status of shipment  EHS will maintain records to comply with regulatory recordkeeping requirements For additional information:  Penn State Hazardous Material Shipping Policy; SY 34http://guru.psu.edu/policies/SY34.html</p><p> Penn State EHS Hazardous Materials Shipping https://ehs.psu.edu/hazardous-materials- shipping/overview</p><p> U.S. Department of Transportation http://www.dot.gov/</p><p> International Air Transport Association http://www.iata.org</p><p>Sept. 21, 2017</p>
