Any shipment containing Dry Ice . Flammable Liquids . Infectious Materials . Biological Specimens . Compressed Gas Cylinders . Other hazardous materials What are the key requirements of this program?  Work units must: o Ensure that individuals shipping hazardous materials, off-site, are aware of and comply with SY 34, Hazardous Materials Shipping  Shipper must: o Provide EHS with: . Name, full address, telephone number, and email address for both sender and recipient. . Detailed description of material to be shipped. . Sender or recipient’s account number for shipping company, i.e. FedEx or UPS. o Provide EHS with approval documentation from Export Control Office for all international shipments.
