See EPID 200: KY Reportable Disease Form</p><p>REPORT IMMEDIATELY by TELEPHONE to the Local Health Department or the KY Department for Public Health:  Unexpected pattern of cases, suspected cases or deaths which may indicate a newly recognized infectious agent  An outbreak, epidemic, related public health hazard or act of bioterrorism, such as SMALLPOX  Outbreaks or Unusual Public Health Occurrences </p><p>Kentucky Department for Public Health in Frankfort Telephone 502-564-3418 or 1-888-9REPORT (973-7678) FAX 502-696-3803</p><p>REPORT WITHIN 24 HOURS Anthrax invasive disease Rabies, human Arboviral Disease* Hansen’s disease Rubella Neuroinvasive Hantavirus infection Rubella syndrome, congenital Non-Neuroinvasive Hepatitis A Salmonellosis Botulism Listeriosis Shigellosis Brucellosis Measles Syphilis, primary, secondary, Campylobacteriosis Meningococcal infections early latent or congenital Cholera Pertussis Tetanus Cryptosporidiosis Plague Tularemia Diphtheria Poliomyelitis Typhoid Fever Escherichia coli (E. coli) 0157:H7 Psittacosis Vibrio parahaemolyticus E. coli shiga toxin positive (STEC) Q Fever Vibrio vulnificus Haemophilus influenzae Rabies, animal Yellow Fever</p><p>REPORT WITHIN ONE (1) BUSINESS DAY</p><p>Animal conditions known to be Hepatitis B infection in a Streptococcal disease communicable to man pregnant woman or invasive, Group A Foodborne outbreak / intoxication a child born in or after 1992 Toxic Shock Syndrome Hepatitis B, acute Mumps Tuberculosis Waterborne outbreak</p><p>REPORT WITHIN FIVE (5) BUSINESS DAYS AIDS HIV infection Rocky Mountain Chancroid Lead poisoning Spotted Fever Chlamydia trachomatis Legionellosis Streptococcus pneumoniae, infection Lyme disease drug-resistant invasive disease Ehrlichiosis Lymphogranuloma venereum Syphilis, other than primary, Gonorrhea Malaria secondary, early latent or Granuloma inguinale Rabies, post exposure congenital Hepatitis C, acute prophylaxis Toxoplasmosis Histoplasmosis</p><p>REPORT WITHIN (3) MONTHS FOLLOWING DIAGNOSIS: Asbestosis, Coal Worker’s Pneumonoconiosis, and Silicosis</p><p>Human Immunodeficiency Virus/AIDS surveillance: See “Report within five (5) business days” above All cases of HIV infections/AIDS are reportable to a separate surveillance system in accordance with KRS 211.180(1)b. To report a HIV/AIDS case call 866-510-0008. </p><p>* Includes California group, Eastern Equine, St. Louis, Venezuelan Equine Western Equine, and West Nile Viruses Laboratory Surveillance: Influenza virus isolates are to be reported weekly by laboratories. 902 KAR 02:065 requires long term care facilities to report an outbreak (2 or more cases) of influenza-like illnesses (ILI) within 24 hours to the local health department or the KDPH.
