We had a good Board to Board session with the Homerton, and are making good progress with the contract for 14/5.</p><p>We need to redo the specification for Community Health Services and this may take some time to negotiate, but we are optimistic that we will get a contract signed earlier than last year.</p><p>Please note physio and foot health activity has gone up a lot since C&B was introduced, so please ensure all clinicians are aware of the need to ensure all referrals are entirely necessary as waiting lists are growing.</p><p>Clare Highton</p><p>Clinical Commissioning Engagement (CCE)</p><p>Please download the final version of Clinical Commissioning and Engagement Contract (previously known as the CC LES) CCE Tracker for 2014-15</p><p>We’ve tried really hard to make everything as clear as possible and bug free but if you do have questions please contact [email protected]</p><p>Sign up</p><p>Sebastian will contact you all next week asking you to sign up, which now has to be via a standard NHS contract. </p><p>Big White Wall</p><p>Leading digital mental health service Big White Wall is now available for everyone aged 16+ Hackney. The service is for anyone experiencing emotional distress and offers: </p><p> Safe, anonymous support</p><p> Trained counsellors online at all times</p><p> Community, group and 1:1 peer therapy</p><p> Guided programmes on smoking, alcohol and weight management</p><p> Clinical tests and a broad range of self-care resources</p><p>The commission has been funded by the NHS Regional Innovation Fund. To join Big White Wall, people simply need to go to www.bigwhitewall.com and enter their Hackney postcode.</p><p>Practices will also be sent publicity materials to display in practice and give to patients.</p><p>More information: see the website or email [email protected]. NHS City and Hackney Clinical Commissioning Group Clinical Executive Committee (CEC) Wednesday 9 April 2014 key issues</p><p>Feedback from the members and CCG Programme Boards</p><p>The CEC requested that Public Health make a presentation to a future meeting to ensure that the CCG plans are consistent across organisations. Consortia Leads raised and discussed issues around year end close processes, Local Enhanced Service (LES) payments, physiotherapy referrals, 2014/15 clinical audits and the community matron service from the Homerton University Hospital NHS Foundation Trust (HUHFT). The future 2014/15 CCG contracts (ex LES services) were also discussed and Consortia Leads informed that practices will be written to, outlining the CCGs plans. The CEC discussed how to approach risk stratification, with the multitude of IT tools and data restrictions and noted that training sessions on the NELIE system are available from the CSU. Read more...</p><p>TIA Service</p><p>Homerton Hospital have informed the CCG that some practices are still using very outdated forms/information when referring patient to the TIA service.</p><p>Please see download the revised TIA Referral Form and the TIA Pathway for Homerton and cascade amongst your teams.</p><p>If you would like any more information please contact the Service Administrator on 020 8510 7577 </p><p>Learning Disabilities Health Check Scheme</p><p>The enhanced service for 2014/15 has been extended to include patients aged 14 and over. We are working with the transition team at Hackney Ark to identify patients aged 14-17yrs who meet the criteria to be included on the learning disabilities "health check" register. These patients will need to be offered an annual health check and health action plan. </p><p>Lucy Gittins, our primary care liaison nurse for learning disabilities will be contacting practices during May 2014 to update you about those patients aged 14-17yrs that will need to be added to the register. Contact Lucy if you have any questions [email protected]</p><p>All practices signed up to this enhanced service will need to attend both of our annual education sessions to meet the requirements of the enhanced service:</p><p>Wednesday May 6th, St Joseph's Hospice, 13:30-15:00 for Reception staff/Practice Managers Wednesday July 9th, Tomlinson Centre, 13:30-15:00 for GPs and Practice Nurses</p><p>Cervical and Breast Screening for our Learning Disabilities population</p><p>Uptake for cervical and breast screening in our learning disabilities population in 2013/14 has been disappointing, only 31% (84) of eligible women were up to date with cervical screening and only 13% (8) of eligible women were up to date with mammography screening.</p><p>Please encourage women to have screening during their annual health checks. Our colleagues in the physio service have reported a significant increase in demand for their service which has led to an increase in waiting times. The Planned Care Board are responsible for commissioning this service and are in discussions with the Locomotor team to look in depth at the issue. In the meantime we would like to ask practices to consider the following actions:</p><p> Are you able to support your patient with advice on appropriate exercise and use of the ARC leaflets?http://www.arthritisresearchuk.org/health-professionals-and- students/information-for-your-patients/exercise-sheets-and-videos.aspx</p><p> Are you familiar with the Locomotor referral form on our website that includes a brief questionnaire about your patients back pain which is useful as part of your decision to refer? http://www.cityandhackneyccg.nhs.uk/Downloads/gp/Forms/Locomotor%20Referral%20Form %20V3%201%20April%202013.doc</p><p>We hope that these short term actions will assist our colleagues in physio in managing referrals. We will also looking at DNA rates and referral rates by practice to establish if there is growth in referrals and any possible trends. We will also be talking with GP colleagues in more detail about this at the Clinical Commissioning Forum on the 1st May.</p><p>Thank you for your support</p><p>Dr Gary Marlowe </p><p>NHS England London Dental Service</p><p>NHS England London Dental Service introduces a more localised Out of Hours Urgent Dental Services across North East London. Out of Hours Urgent dental treatment services are available at five locations across North East London. The area covered is shown in red on the map. </p><p>To access these services you must telephone 020 3594 0938 and a dental professional will then assess your dental needs over the phone.</p><p>If you require urgent dental treatment, you will be offered an appointment as near to you as possible, either the same day or the following day.</p><p>The telephone lines will be open Monday to Friday 18:00 to 21:00 hours and Saturday, Sunday and Bank Holidays 09:00 hours to 14:00 hours. For especially busy periods (such as some Bank Holidays), phone lines will be open between 09:00 and 16:00 hours.</p><p>Download the poster </p><p>Education & Training</p><p>The Friday education events at Homerton return on April 25th the medicines management team will be introducing GPs to the components of the Clinical Commissioning and Engagement Contract for 14/15. The next CCE (formerly CC LES) education event is on 9th May. There is mounting evidence that it can be of benefit in managing the stress of cancer.</p><p>There is a new course starting on 25th April 2014. Please see the information for patients and referrer information about who and how to refer.</p><p>This course is for patients who have advanced cancer, on-going physical effects or high risk of recurrence. </p><p>ParaDoc Service - Paramedic and GP response unit</p><p>ParaDoc Service - Paramedic and GP response unit in City and Hackney launched on Friday 28th March 2014.</p><p>The ParaDoc service is launching this week. GPs will play a vital role in making ParaDoc a success.</p><p>This new and innovative service will consist of a paramedic and experienced GP working together to respond to patients with urgent healthcare needs within the community setting.
